Unraveling the optimum latent structure of attention-deficit/hyperactivity disorder : evidence supporting ICD and HiTOP frameworks

- Description: Attention Deficit/hyperactivity disorder (ADHD) is conceptualized differently in the Diagnostic and Statistical Manual (DSM-5), the International Classification of Diseases-10 (ICD-10), and the Hierarchical Taxonomy of Psychopathology (HiTOP) frameworks. This study applied independent cluster confirmatory factor analysis (ICM-CFA), exploratory structure equation model with target rotation (ESEM), and the S-1 bi-factor CFA approaches to evaluate seven ADHD models yielded by different combinations of these taxonomic frameworks. Parents and teachers of a community sample of children (between 6 and 12 years of age) completed the Disruptive Behavior Rating Scale (for ADHD symptoms) and the Strengths and Difficulties Questionnaire (for validation). Our findings for both parent and teacher ratings provided the most support for the S-1 bi-factor CFA model comprised of (i) a g-factor based on ICD-10 impulsivity symptoms as the reference indicators and (ii) inattention and hyperactivity as specific factors. However, the hyperactivity-specific factor lacked clarity and reliability. Thus, our findings indicate that ADHD is best viewed as a disorder primarily reflecting impulsivity, though with a separable inattention (but no hyperactivity) component, i.e., “ADID (attention deficit/impulsivity disorder).” This model aligns with the HiTOP proposals. © Copyright © 2021 Gomez, Liu, Krueger, Stavropoulos, Downs, Preece, Houghton and Chen.

- Description: Objective: To examine the factor structure of the Sleep Disorder Scale for Children (SDSC) in children and adolescents with attention deficit and hyperactivity disorder (ADHD). Method: The caregivers of 307 children with ADHD completed the SDSC. Standard and bifactor confirmatory factor analysis (CFA) evaluated the goodness-of-fit of competing factor structures. Results: The original and unidimensional factor structure produced sub-optimal fit. Bifactor exploratory factor analysis (EFA) was performed to examine the underlying structure of the SDSC. A revised bifactor solution comprising six-specific factors and a general factor was identified. A nested version of this model was deemed to be the preferred model, which also demonstrated good psychometric properties. Conclusion: There is evidence of a ‘general sleep difficulties’ factor in children with ADHD. Four of the six original factors were replicated in this study. However, the revised factor structure suggests that clinicians should be cautious of the utility of subscale scores pending further validation in ADHD samples. © 2019

- Description: Receiver operating characteristic curve analysis was used to examine and compare the diagnostic accuracy of the Conners 3–Parent Short Form (C 3-P(S)), and the Conners 3–Teacher Short Form (C 3-T(S)) inattention and hyperactivity/impulsivity scales, and the Child Behavior Checklist (CBCL) and Teacher’s Report Form (TRF) attention problems scales, to distinguish those with and without attention deficit/hyperactivity disorder (ADHD). It also examined and compared the diagnostic accuracy of the C 3-P(S) and C 3-T(S) Aggression (AG) scales, and the CBCL and TRF Aggressive Behavior (AB) scales, to distinguish those with and without oppositional defiant disorder (ODD). The study used archival data (N = 150-261) involving a large group of clinic-referred children aged between 6 and 11 years who had been interviewed for clinical diagnosis of ADHD and ODD using the Anxiety Disorders Interview Schedule for Children (ADISC-IV) as the reference standard, and then administered one or more of the screening measures. The findings provided empirical support for the use of the C 3-P(S) and CBCL for identifying ADHD and ODD, with the CBCL aggressive behavior scale having better ability to detect ODD. The implications of the findings for using the screening scales for diagnoses of ADHD and ODD are discussed. © The Author(s) 2019.

- Description: A meta-analysis of up to 20 datasets is reported that examined the relationships between Cloninger's personality dimensions and Attention Deficit Hyperactivity Disorder (ADHD). Cloninger's model comprises four temperament (Novelty-Seeking, Harm-Avoidance, Reward Dependence, and Persistence) and three character (Self-Directedness, Cooperation, and Self-Transcendence) dimensions. Major findings were that all personality dimensions, except Self-Transcendence, were significantly associated with ADHD. These associations had different directions and magnitudes of relationship with ADHD. The associations were especially strong for Novelty-Seeking and Self-Directedness, being positive for Novelty-Seeking and negative for Self-Directedness. In addition, the association between ADHD and Persistence was moderated by age (stronger in children than adults) and source (stronger in clinical samples than community samples). Further, the association between harm avoidance and ADHD was moderated by age (strong and significant in adults, but not significant in children). Findings are discussed in relation to past work and the different strengths of the associations found between Cloninger's personality dimensions and ADHD, developmental differences in these relationships, implications for theoretical models of ADHD, the influence of biological and environmental factors in the expression of ADHD, implications for treatment and diagnosis, and the underlying processes for the relationships between personality and ADHD. © 2016 Elsevier Ltd

- Description: OBJECTIVE: ADHD and Hyperkinetic Disorder (HKD) have the same 18 symptoms, covering inattention (IA), hyperactivity (HYP), and impulsivity (IMP). This study was aimed at providing descriptive scores for the different symptom groups in these disorders and how these scores varied by age and gender, the percentages of individuals meeting the symptom thresholds for the different ADHD types and HKD, the factor structure, and gender invariance of these symptoms in adults. METHOD: To accomplish this, 852 adults provided self-ratings for a scale comprising the 18 ADHD/HKD symptoms. RESULTS: The findings showed that age and gender had minimal effects on the ADHD symptoms. Also, in terms of symptom counts, 2% had HKD and 6.3% had ADHD (inattentive = 1.6%, hyperactive-impulsive = 2.7%, and combined type = 2.0%). Confirmatory factor analysis (CFA) provided most support for the three-factor HKD model, involving separate factors for the IA, HYP, and IMP symptoms. This model showed full measurement invariance across gender. CONCLUSION: The theoretical and clinical implications of the findings are discussed.

- Description: OBJECTIVE: This study used confirmatory factor analysis (CFA), latent class analysis (LCA), and factor mixture model (FMM) to ascertain the best model for ADHD symptoms, as presented in the Strengths and Weaknesses of ADHD-Symptoms and Normal Behavior Scale (SWAN). METHOD: Mother ratings of the ADHD symptoms of 418 clinic-referred children and adolescents were obtained. RESULTS: The findings provided most support for an FMM with two classes (less severe inattention [IA] and hyperactivity/impulsivity [HI] symptoms and more severe IA/HI symptoms) and two factors (IA and HI) model. The more severe class had more individuals with the ADHD diagnosis, especially the combined type. CONCLUSION: The findings are discussed in relation to dimensional and categorical views of ADHD.

- Description: Objective: This study used confirmatory factor analysis to examine the factor structure for the 10 core WISC-IV subtests in a group of children (N = 812) with ADHD. Method: The study examined oblique four- and five-factor models, higher order models with one general secondary factor and four and five primary factors, and a bifactor model with a general factor and four specific factors. Results: The findings supported all models tested, with the bifactor model being the optimum model. For this model, only the general factor had high explained common variance and omega hierarchical value, and it predicted reading and arithmetic abilities. Conclusion: The findings favor the use of the FSIQ scores of the WISC-IV, but not the subscale index scores. © 2016 Gomez, Vance and Watson.

- Description: This study examined the level of agreement of adolescent ratings with mother ratings, and adolescent ratings with teacher ratings of the inattention (IA) and hyperactivity/impulsivity (HI) symptom groups of ADHD. A total of 214 adolescents provided self-ratings of IA and HI, and their IA and HI were also rated by their mothers and teachers. The correlated trait-correlated method minus one model was applied, with adolescent ratings as the reference method, and the other two ratings as the non-reference methods. The findings indicated no additional variance in adolescent ratings for IA and HI that could not be accounted by mother ratings of IA and HI, respectively. In contrast, there was additional variance in adolescent ratings for IA and HI that could was not accounted by teacher ratings of IA and HI, respectively. The findings suggest that when diagnosing ADHD in adolescents, their reports of their own ADHD behaviors are not needed when mother reports of such behaviors are used. © 2015.

- Description: We report a meta-analysis of up to 40 data sets that examined the personality dimensions in the Five-Factor Model (FFM) and the integrated Five-Factor Model (IFFM) in relation to ADHD symptom domains of inattention (IA) and hyperactivity/impulsivity (HI). The IFFM incorporated the dimensions of other personality models (in particular, those of Eysenck, Tellegen, and Cloninger, as well as the FFM). Major findings were: (1) IA and HI were both associated with low conscientious inhibition/conscientiousness, and low agreeable inhibition/agreeableness, and with high negative emotionality/neuroticism; (2) conscientious inhibition and conscientiousness were more strongly related to IA than HI; (3) agreeable inhibition and agreeableness were more strongly related to HI than IA; and (4) the association of conscientious inhibition and conscientiousness with HI was moderated by age group and source from where participants were recruited (associations were stronger in children than adults, and clinical samples than community samples). These findings are discussed in relation to single and multiple pathway theories, underlying factors and processes for the personality-ADHD link, and clinical implications.

- Description: Relationships between Rothbart's 13 temperament sub-dimensions and the Attention-Deficit/Hyperactivity Disorder (ADHD) factors for the 2-factor model [inattention (IA) and hyperactivity/impulsivity (HI) domains] and the bifactor model (general ADHD, and specific factors for IA and HI) were examined in 267 adults from the general population. Regression analyses revealed that (1) both the IA and HI factors in the 2-factor model and the general ADHD factor in the bifactor model were predicted positively by sad, discomfort and associative sensitivity, and negatively by activation control, (2) the HI domain factor in the 2-factor model was also predicted negatively by inhibitory control, (3) the specific IA factor in the bifactor model was predicted negatively by activation control and attention control, and (4) the HI specific factor in the bifactor model was predicted negatively by inhibitory control and positively by sociability. - Description: Objective: This study examined the bifactor models of ADHD in three groups: adolescents from the general community (n = 214), adults from the general community (n = 366), and a clinic sample (n = 245). Method: For the adolescent sample, mothers provided ratings of the ADHD symptoms, and their teachers provided ratings for Emotional Symptoms, Conduct Problems, Hyperactivity, Peer Problems, and Prosocial Behavior. For the adult sample, all adults completed self-ratings of the ADHD symptoms and measures for depression, anxiety, and stress. For the clinic-referred sample, mothers provided ratings of the ADHD symptoms, and they were diagnosed for ADHD, Conduct Disorder/Oppositional Defiant Disorder, anxiety disorders, and depression disorders. Results: The findings for all samples supported the bifactor model, and there were differences in the way the general and specific factors correlated with the external variables. Cloninger's temperament and character dimensions and the Attention-Deficit/Hyperactivity Disorder (ADHD) symptoms of inattention (IA) and hyperactivity/impulsivity (HI) were examined in 231 adults from the general population. Results showed that IA was associated positively with Sensitivity to Punishment (in the SPSRQ), Negative Emotionality (in the MPQ-BF), and Behavioral Inhibition System (in the BIS/BAS scales). HI was associated positively with Sensitivity to Reward (in the SPSRQ), Reward Responsiveness and Drive (both in the BIS/BAS scales), and Positive Emotionality (in the MPQ-BF). Both IA and HI were associated negatively with Constraint (in the MPQ-BF), and HI was associated positively with Fun Seeking (in the BIS/BAS scales). - Description: Objective: The aims of the study were to examine differential symptom functioning (DSF) and agreement across parent and teacher ratings for the DSM-IV ADHD inattention (IA) and hyperactivity/impulsivity (HI) symptoms, listed in the Disruptive Behavior Rating Scale (DBRS). Method: DSF was examined using a parametric technique involving ordinal logistic regression, and also a nonparametric technique known as kernel smoothing implemented in the program TestGraf. To accomplish the aims, Australian parents and teachers completed the DBRS for 213 children, between 6 and 11 years of age. Results: The results of both analytical procedures showed no DSF across all the IA and HI symptoms. Additional results indicated low parent-teacher agreement. Conclusion: The absence of DSF suggests that previous reports of low parent-teacher agreement are likely to reflect a true state of affairs and that parent and teacher ratings can be compared. In this context, the low parent-teacher agreement can be interpreted as indication that the attention deficit hyperactivity disorder (ADHD) symptoms are situation specific. (J. of Att. - Description: The study used the ordinal logistic regression method to examine gender differential item functioning (DIF) for ratings of the ADHD symptoms. This was examined for two different response scoring methods: ordinal and binary. Parents from the general community completed ratings for 742 and 733 primary school-aged girls and boys, respectively. Results indicate no gender DIF for all the ordinal scored inattention (IA) and hyperactivity/impulsivity (HI) symptoms, and all the binary scored IA symptoms. For the binary scored HI symptoms, there was DIF for one symptom, while DIF could not be computed for three symptoms. Overall, these findings suggest minimal gender DIF for the ADHD symptoms. The implications of the findings for the use of ADHD rating scales in clinical practice are discussed. © 2006.

- Description: The purpose of this study was to examine the measurement (configural, metric, scalar, and residual) and structural (factor variance, factor covariance, and factor means) invariance of parent ratings of the attention-deficit/hyperactivity disorder-inattention (ADHD-IN), ADHD-hyperactivity/impulsivity (ADHD-HI), and oppositional defiant disorder (ODD) symptoms as described in the Diagnostic and Statistical Manual of Mental Disorders (4th ed.; American Psychiatric Association, 1994) across boys and girls. In an American pediatric sample (N = 1,015) and a Malaysian elementary school-age sample (N = 928), there was strong support for configural, metric, scalar, residual, factor variance, and covariance invariance across gender within each sample. Both American and Malaysian boys had significantly higher scores on the ADHD-IN and ADHD-HI factor means than did girls, whereas only in the American sample did boys score significantly higher on the ODD factor than did girls. The three trait factors were ADHD-IN, ADHD-HI, and ODD. The two source factors were parents and teachers.
