- Description: In this paper global optimisation-based techniques are studied in order to increase the accuracy of medical diagnosis and prognosis with FNA image data from the Wisconsin Diagnostic and Prognostic Breast Cancer databases. The design of three dimensional environments rich in narrative and character depth can be facilitated with a detailed representation of interactions between characters. However, the representation of interaction in current 3D development environments such as game engines is quite basic. This work advances a scheme for representing interactions that integrates a representation of semantics from linguistics called FrameNet with conceptualizations of drama and narrative by Georges Polti and Joseph Campbell. - Description: This research presents a classifier that aims to provide insight into a dataset in addition to achieving classification accuracies comparable to other algorithms. The classifier called, Automated Weighted Sum (AWSum) uses a weighted sum approach where feature values are assigned weights that are summed and compared to a threshold in order to classify an example. Though naive, this approach is scalable, achieves accurate classifications on standard datasets and also provides a degree of insight. By insight we mean that the technique provides an appreciation of the influence a feature value has on class values, relative to each other. - Description: The concept of dramatic level is crucial for a model of dramatic flow. We present a framework to maintain optimal dramatic flow in an interactive 3D environment where both linear and emergent narratives co-exist. Unlike all other interactive narrative prototypes the framework advanced focuses on the optimal dramatic flow of the emerging user narrative so that although fragmented, it can be engaging and make sense. In university degree programs, students and novice diagnosticians cannot have the exposure to cases possible in the traditional context. An online system that engages students in the interactive construction of a virtual case and provides immediate feedback on the appropriateness of student actions and the accuracy of diagnostic conclusions can enhance student learning. The system, an Intelligent Interactive Learning Environment (IILE) is based on an approach that has been shown to improve learning outcomes in intensive care nurse training. Methods: An expert model of diagnostic reasoning elicited from TCM expert practitioners lies at the core of the IILE. The knowledge acquisition is performed using an argumentation tree representation that has been shown to be effective in structuring complex knowledge and facilitating engineer - expert interactions. Problems associated with keeping knowledge bases up to date are mitigated with the use of a knowledge model known as ripple down rules permits dynamic updating of knowledge so that knowledge bases evolve over time. A simple narrative model builds up the virtual case study as user interaction proceeds. - Description: Data mining is often performed with datasets associated with diseases in order to increase insights that can ultimately lead to improved prevention or treatment. Classification algorithms can achieve high levels of predictive accuracy but have limited application for facilitating the insight that leads to deeper understanding of aspects of the disease. This is because the representation of knowledge that arises from classification algorithms is too opaque, too complex or too sparse to facilitate insight. Clustering, association and visualisation approaches enable greater scope for clinicians to be engaged in a way that leads to insight, however predictive accuracy is compromised or non-existent. This research investigates the practical applications of Automated Weighted Sum, (AWSum), a classification algorithm that provides accuracy comparable to other techniques whilst providing some insight into the data. This is achieved by calculating a weight for each feature value that represents its influence on the class value. Clinicians are very familiar with weighted sum scoring scales so the internal representation is intuitive and easily understood. This paper presents results from the use of the AWSum approach with data from patients suffering from Cystic Fibrosis.

The trend toward co-existence of medical systems presents challenges for health informatics including the need to develop standards that can encompass the diversity required, the need to develop software applications that effectively inter-operate across diverse systems and the need to support patients when evaluating competing systems. This article advances the notion that the challenges can most effectively be met with the development of informatics approaches that do not assume the superiority of one medical system over another. - Description: Cardiovascular autonomic neuropathy (CAN) is one of the important causes of mortality among diabetes patients. Statistics shows that more than 22% of people with type 2 diabetes mellitus suffer from CAN and which in turn leads to cardiovascular disease (heart attack, stroke). Therefore early detection of CAN could reduce the mortality. Traditional method for detection of CAN uses Ewing's algorithm where five noninvasive cardiovascular tests are used. Often for clinician, it is difficult to collect data from for the Ewing Battery patients due to onerous test conditions. In this paper, we propose a hybrid of wrapper-filter approach to find novel features from patients' ECG records and then generate decision rules for the new features for easier detection of CAN. In the proposed feature selection, a hybrid of filter (Maximum Relevance, MR) and wrapper (Artificial Neural Net Input Gain Measurement Approximation ANNIGMA) approaches (MR-ANNIGMA) would be used. The combined heuristics in the hybrid MRANNIGMA takes the advantages of the complementary properties of the both filter and wrapper heuristics and can find significant features. The selected features set are used to generate a new set of rules for detection of CAN. Experiments on real patient records shows that proposed method finds a smaller set of features for detection of CAN than traditional method which are clinically significant and could lead to an easier way to diagnose CAN. © 2010 IEEE.

- Description: Wireless sensor devices are used for monitoring patients with serious medical conditions. Communication of content-sensitive and context sensitive datasets is crucial for the survival of patients so that informed decisions can be made. The main limitation of sensor devices is that they work on a fixed threshold to notify the relevant Healthcare Professional (HP) about the seriousness of a patient’s current state. Further, these sensor devices have limited processor, memory capabilities and battery. A new grid-based information monitoring architecture is proposed to address the issues of data loss and timely dissemination of critical information to the relevant HP. The proposed approach provides an opportunity to efficiently aggregate datasets of interest by reducing network overhead and minimizing data latency. To narrow down the problem domain, in-network processing of datasets with Grid monitoring capabilities is proposed for the efficient execution of the computational, resource and data intensive tasks. Interactive wireless sensor networks do not guarantee that data gathered from the heterogeneous sources will always arrive at the sink (base) node, but the proposed aggregation technique will provide a fault tolerant solution to the timely notification of a patient’s critical state. Experimental results received are encouraging and clearly show a reduction in the network latency rate.

- Description: Treatment refers to the therapy to treat a disease or a health issue. Treatment in this situation is similar to medical treatment which mainly uses medicines in an attempt to relieve the pain or even stop the disease. However, medicines themselves could not entirely cure the disease (in this case, diabetes), the patients will need more intervention which will be introduced in the next section. In most of documents for diabetic treatment, insulin therapy may be the main factor, however it would seem that diabetic patient needs more than just insulin. Therefore, TCM – traditional Chinese medicine – is recommended in the diabetic treatment as a lot of its remedies not only adjust insulin but also maintain good health for the patients. This section presents some of the TCM remedies to treat diabetes. As mentioned, diabetic patients are treated by lifestyle intervention and insulin therapy according to their diabetic status. The prevalence of diabetes and its complications leads to the requirement of treatment and care plan. Guidelines for T2D treatment indicated the following primary areas: lifestyle improvement which involves at least two and half hours of physical operations every week, dietary plan which decreases the fat intake, and weight management which requires weight loss approximately 7% of the baseline weight; cardiovascular risk factor reduction by managing blood pressure, cholesterol level, control smoking status, hypertension; and blood glucose management such as mono-therapy methods using oral medications to reduce A1c levels (Ripsin, Kang, & Urban, 2009). Self-monitoring of blood glucose levels for T2D treatment is also suggested. The self-monitoring of blood glucose method is recommended because it could enhance the patients’ self-consciousness of managing their diabetic status and require greater behaviours, responsibilities and efforts. Besides, this method is cost-effective in long term for diabetic complications treatment (Szymborska-Kajaneka, Psureka, Heseb, & Strojek, 2009). Another related study recommended that for T2D patients who are using insulin, self-monitoring of blood glucose should be carried out daily at least three times; and for patients without insulin usage the frequency of blood glucose self-monitoring should be adjusted individually (Varanauskiene, 2008). Both studies indicate that there have been controversies whether self-monitoring of blood glucose is useful for T2D patients without insulin treatment. We recommend traditional Chinese medicine (TCM) as the major medicine for treating diabetes according to a report of natural Chinese medicines (Li, Zheng, Bukuru, & Kimpe, 2004) which indicates the results from many cases in various research and medical activities.

- Description: Clinical decision-support is gaining widespread attention as medical institutions and governing bodies turn towards utilising better information management for effective and efficient healthcare delivery and quality assured outcomes. Amass of data across all stages, from disease diagnosis to palliative care, is further indication of the opportunities and challenges created for effective data management, analysis, prediction and optimization techniques as parts of knowledge management in clinical environments. A Data-driven Decision Guidance Management System (DD-DGMS) architecture can encompass solutions into a single closed-loop integrated platform to empower clinical scientists to seamlessly explore a multivariate data space in search of novel patterns and correlations to inform their research and practice. The paper describes the components of such an architecture, which includes a robust data warehouse as an infrastructure for comprehensive clinical knowledge management. The proposed DD-DGMS architecture incorporates the dynamic dimensional data model as its elemental core. Given the heterogeneous nature of clinical contexts and corresponding data, the dimensional data model presents itself as an adaptive model that facilitates knowledge discovery, distribution and application, which is essential for clinical decision support. The paper reports on a trial of the DD-DGMS system prototype conducted on diabetes screening data which further establishes the relevance of the proposed architecture to a clinical context.
