Description: Objectives: To obtain benefits from sports injury prevention programs, players are instructed to perform the exercises as prescribed. We developed an observational checklist to measure the quality of exercise performance by players participating in FootyFirst, a coach-led, exercise-based, lower-limb injury prevention program in community Australian Football (AF). Design: Observational. Methods: The essential performance criteria for each FootyFirst exercise were described in terms of the technique, volume and intensity required to perform each exercise. An observational checklist was developed to evaluate each criterion through direct visual observation of players at training. The checklist was trialled by two independent raters who observed the same 70 players completing the exercises at eight clubs. Agreement between observers was assessed by Kappa-statistics. Exercise fidelity was defined as the proportion of observed players who performed all aspects of their exercises correctly. Results: The raters agreed on 61/70 observations (87%) (Kappa = 0.72, 95% CI: 0.55; 0.89). Of the observations with agreed ratings, 41 (67%) players were judged as performing the exercises as prescribed. Conclusions: The observational checklist demonstrated high inter-rater reliability. Many players observed did not perform the exercises as prescribed, raising concern as to whether they would be receiving anticipated program benefits. Where quality of exercise performance is important, evaluation and reporting of program fidelity should include direct observations of participants.

Description: The study aim was to evaluate the implementation of group-and home-based exercise falls prevention programs delivered through community health agencies to community-dwelling older people. Interviews with program staff were guided by the Diffusion of Innovations theory. Highly consistent themes emerged for the two types of programs. Both had high overall compatibility, high relative advantage, good observability and high inherent trialability-all factors known to strengthen implementation. The level of complexity and low financial compatibility emerged as the strongest potential inhibitors to program implementation in the context examined. The two main factors contributing to complexity were the need to challenge balance safely across a broad range of capability, and practical considerations associated with program delivery. A range of strategies to provide more technical support for exercise program leaders to tailor balance challenge for exercise program leaders may enhance implementation of falls prevention exercise programs. © 2016 Human Kinetics, Inc.

Description: Prostate cancer (PCa) is the second most frequent malignancy in men worldwide, accounting for 27% of new cancer cases in men and responsible for 3.8% of worldwide male cancer deaths (2018). This risk is projected to double before 204, so it is surprising that there are no well- established modifiable risk factors for PCa diagnosis orr progression. Considering this, the following thesis consists of three original research studies that examined; (1) the moderating effect of physical activity and muscular strength on prostate cancer mortality; (2) the magnitude of effect from pooled therapeutic exercise studies to impact quality of life and adverse events in PCa patients following surgery; (3) the head-to-head comparative efficacy of individual exercise therapies to impact quality of life and adverse events following surgery. The sum of works presented in this thesis lead to the conclusion that 1) when PCa is the only lifetime cancer diagnosis, life expectancy is similar to lifetime cancer free counterparts whereas diagnosis of PCa with any other lifetime cancer is associated with a 30.2% lower life expectancy during 10-year follow-up. Physical activity has negligible effect on life-expectancy whereas handgrip strength accounts for ~4% of the difference in life-expectancy between PCa with any other lifetime cancer compared with controls; 2) Pooled evidence from randomised controlled trials (n=999 participants) identifies that perioperative exercise therapy (aerobic, resistance, pelvic floor, mind-body exercise, or mixed exercise (combined aerobic and resistance) has a positive impact on patient quality of life and adverse events in PCa patients undergoing surgery which is predominant in perceptions of symptom related QoL (SMD: 0.56, 111 95% CI’s [0.22-0.89]) more so than health related quality of life (HRQoL) (SMD: 0.02, 95% 112 CI’s [-0.16-0.20]; 3) Comparison of head-to-head efficacy of different therapeutic exercise interventions [Aerobic, resistance, pelvic floor, mind-body exercise or mixed exercise (combined aerobic and resistance)] identified pelvic floor muscle exercise to be the most favorable exercise therapy to maintain QoL in PCa patients undergoing surgery, partly due to the current lack of evidence to support other exercise therapies. These findings provide new knowledge to support the effective treatment of PCa patients.

Description: Sport is a common context for injury. It is the most common reason for hospital-treated injury in adolescents and young adults and there is some evidence that injury rates at the population level are increasing. Sports injuries can occur to participants across all forms of sport ranging from elite/professional sport to competitive sport in clubs/colleges/schools to school sport to a range of fitness and physical activity programs usually undertaken for health and social reasons. Over recent years, evidence has accumulated that the majority of these injuries should be preventable if sports injury interventions are successfully implemented. The challenge remains to demonstrate the effectiveness of many sports injury interventions in appropriate real-world settings and to better understand the drivers and barriers to sports injury prevention implementation efforts. © 2014 Elsevier Inc.
