- Description: There is accumulating international evidence that lower limb injuries in sport can be prevented through targeted training but the extent to which this knowledge has been translated to real-world sporting practice is not known. A semi-structured questionnaire of all coaches from the nine Sydney Australian Football League Premier Division teams was conducted. Information was sought about their knowledge and behaviours in relation to delivering training programs, including their uptake of the latest scientific evidence for injury prevention. Direct observation of a sample of the coach-delivered training sessions was also undertaken to validate the questionnaire. Coaches ranked training session elements directly related to the game as being of most importance. They strongly favoured warming-up and cooling-down as injury prevention measures but changing direction and side-stepping training was considered to be of little/no importance for safety. Only one-third believed that balance training had some importance for injury prevention, despite accumulating scientific evidence to the contrary. Drills, set play, ball handling and kicking skills were all considered to be of least importance to injury prevention. These views were consistent with the content of the observed coach-led training sessions. In conclusion, current football training sessions do not give adequate attention to the development of skills most likely to reduce the risk of lower limb injury in players. There is a need to improve the translation of the latest scientific evidence about effective injury prevention into coaching practices. © 2008 Sports Medicine Australia.

During the pre-season, 485 players from the elite Victorian under-18 Australian Rules football competition were tested for height, body mass, hand span, arm length, standing reach, vertical jump, 5 and 20 m sprint times, agility, predicted over(V, ) O2 max and sit and reach flexibility. Performance indicators included being selected for the first game of the season and the number of possessions, marks, hitouts, and the number of games where votes were awarded in the first eight games of the season. The top and bottom four teams on the ladder were also compared after eight games. Players were divided into groups on the basis of the above indicators and the groups were compared statistically by analysis of variance (ANOVA). There were several significant differences between selected and non-selected players. Players acquiring the most possessions were significantly (p < 0.05) shorter (effect size: ES = 0.78), with less body mass (ES = 0.55) and possessed greater acceleration (ES = 0.44-0.56) and endurance (ES = 0.51). Body mass was significantly related to the number of marks and height was related to hitouts (p < 0.05). Acceleration was the only fitness quality to discriminate between higher and lower vote winners. The players from the top four teams had a significantly greater standing reach (p = 0.038, ES = 0.53), were heavier (p = 0.032, ES = 0.55) but not superior in any fitness measure. Generally hand span, agility and flexibility were not related to performance indicators. It was concluded that shorter and lighter players who possess high levels of speed and endurance are more likely to acquire possessions and be awarded votes, however these attributes do not guarantee team success. The small relationships between agility and flexibility to performance might be explained by the choice of tests used to assess these qualities. © 2006 Sports Medicine Australia.

Design: Assessment of intercoder agreement. Setting: Community Australian football. Participants: 1082 standardised injury surveillance records. Main outcome measurements: Direct comparison of the four-character hierarchical OSICS-10 codes assigned by two independent coders (a sports physician and an epidemiologist). Adjudication by a third coder (biomechanist). Results: The coders agreed on the first character 95% of the time and on the first two characters 86% of the time. They assigned the same four-digit OSICS-10 code for only 46% of the 1082 injuries. The majority of disagreements occurred for the third character; 85% were because one coder assigned a non-specific 'X' code. The sports physician code was deemed correct in 53% of cases and the epidemiologist in 44%. Reasons for disagreement included the physician not using all of the collected information and the epidemiologist lacking specific anatomical knowledge. Conclusions: Sports injury research requires accurate identification and classification of specific injuries and this study found an overall high level of agreement in coding according to OSICS-10. The fact that the majority of the disagreements occurred for the third OSICS character highlights the fact that increasing complexity and diagnostic specificity in injury coding can result in a loss of reliability and demands a high level of anatomical knowledge. The vast majority of Australian football ovals are transformed into cricket pitches during the football off-season and hence the characteristics of both sports had to be duly considered in the development of standards that could be tested in a laboratory setting, for a synthetic turf surface. This paper describes the data collection and test methods undertaken in the development of the standards for synthetic surface use in Australian football and cricket. The paper also discusses the issues and challenges encountered during the development of standards for multi-sport synthetic surfaces to ensure player safety while maintaining the performance characteristics of both sports. Surface property and ball interaction tests were undertaken on natural playing surfaces, both in situ and in the laboratory to determine the properties of the current playing surface for each sport. This paper highlights the importance of careful consideration of the characteristics of both games and the use of equipment from both sports in the testing methods. The standards described in this paper have now been accepted by the Australian Football League and Cricket Australia and the product approval process and use of synthetic surfaces for Australian football and cricket is imminent.

There are numerous settings where injury surveillance is not mandatory and having a better understanding of the barriers to conducting injury surveillance would lead to improved implementation strategies. One such setting is community sport, where a lack of available epidemiological data has impaired efforts to reduce injury. This study aimed to i) evaluate use of an injury surveillance system following delivery of an implementation strategy; and ii) investigate factors influencing the implementation of the system in community sports clubs. METHODS:A total of 78 clubs were targeted for implementation of an online injury surveillance system (approximately 4000 athletes) in five community Australian football leagues concurrently enrolled in a pragmatic trial of an injury prevention program called FootyFirst. System implementation was evaluated quantitatively, using the RE-AIM framework, and qualitatively, via semi-structured interviews with targeted-users. RESULTS:Across the 78 clubs, there was 69% reach, 44% adoption, 23% implementation and 9% maintenance. Reach and adoption were highest in those leagues receiving concurrent support for the delivery of FootyFirst. Targeted-users identified several barriers and facilitators to implementation including personal (e.g. belief in the importance of injury surveillance), socio-contextual (e.g. understaffing and athlete underreporting) and systems factors (e.g. the time taken to upload injury data into the online system). CONCLUSIONS:The injury surveillance system was implemented and maintained by a small proportion of clubs. Outcomes were best in those leagues receiving concurrent support for the delivery of FootyFirst, suggesting that engagement with personnel at all levels can enhance uptake of surveillance systems. Interview findings suggest that increased uptake could also be achieved by educating club personnel on the importance of recording injuries, developing clearer injury surveillance guidelines, increasing club staffing and better remunerating those who conduct surveillance, as well as offering flexible surveillance systems in a range of accessible formats. By increasing the usage of surveillance systems, data will better represent the target population and increase our understanding of the injury problem, and how to prevent it, in specific settings.

- Description: In sports injury prevention field trials, data collectors are often club volunteers with considerable knowledge of the game but with limited detailed medical backgrounds or knowledge of formal scientific processes. The aim of this paper is to determine the agreement among trained primary data collectors (PDCs) with a sport science background and no prior involvement in data collection in a large randomised controlled trial. During the 'Preventing Australian Football Injury through eXercise' (PAFIX) project, player participation and injury data were collected by trained PDCs at training and games over the 2007 and 2008 playing seasons in 40 community level Australian football teams. PDC-collected data relating to player exposure and whether or not a player sustained an injury and subsequently left the field of play was compared to the same information from independent observers (IOs) who attended one randomly selected game for each of the 40 teams. There was 98% agreement between the PDC and the IO on game details (i.e., date, time, grade and score), 79% (ICC 0.9, 95% CI 0.85-0.95) agreement on the number of players per game and 76% (ICC 0.8, 95% CI 0.69-0.91) agreement on the number of injuries sustained in the games. There was 100% agreement on whether the player left the field for all injuries. - Description: Objective:High-quality sport-specific information about the nature, type, cause, and frequency of injuries is needed to set injury prevention priorities. This article describes the type, nature, and mechanism of injuries in community Australian Football (community AF) players, as collected through field-based monitoring of injury in teams of players.Data Sources:Compilation of published prospectively collected injury data from 3 studies in junior community AF (1202 injuries in 1950+ players) and 3 studies in adult community AF (1765 injuries in 2265 players). This was supplemented with previously unpublished data from the most recent adult community AF injury cohort study conducted in 2007 to 2008. Injuries were ranked according to most common body regions, nature of injury, and mechanism.Main Results:In all players, lower limb injuries were the most frequent injury in community AF and were generally muscle strains, joint sprains, and superficial injuries. These injuries most commonly resulted from incidental contact with other players, or from overexertion. Upper limb injuries were less common but included fractures, strains, and sprains that were generally caused by incidental contact between players and the result of players falling to the ground.Conclusions:Lower limb injuries are common in community AF and could have an adverse impact on sustained participation in the game. Based on what is known about their mechanisms, it is likely that a high proportion of lower limb injuries could be prevented and they should therefore be a priority for injury prevention in community AF.

- Description: Objective:High-quality sport-specific information about the nature, type, cause, and frequency of injuries is needed to set injury prevention priorities. This article describes the type, nature, and mechanism of injuries in community Australian Football (community AF) players, as collected through field-based monitoring of injury in teams of players.Data Sources:Compilation of published prospectively collected injury data from 3 studies in junior community AF (1202 injuries in 1950+ players) and 3 studies in adult community AF (1765 injuries in 2265 players). This was supplemented with previously unpublished data from the most recent adult community AF injury cohort study conducted in 2007 to 2008. Injuries were ranked according to most common body regions, nature of injury, and mechanism.Main Results:In all players, lower limb injuries were the most frequent injury in community AF and were generally muscle strains, joint sprains, and superficial injuries. These injuries most commonly resulted from incidental contact with other players, or from overexertion. Upper limb injuries were less common but included fractures, strains, and sprains that were generally caused by incidental contact between players and the result of players falling to the ground.Conclusions:Lower limb injuries are common in community AF and could have an adverse impact on sustained participation in the game. Based on what is known about their mechanisms, it is likely that a high proportion of lower limb injuries could be prevented and they should therefore be a priority for injury prevention in community AF.

Methods: 112 ground hardness assessments were undertaken using a Clegg hammer at nine locations across 20 grounds, over the 2007 and 2008 AF seasons. Details of 352 injuries sustained by community level players on those grounds were prospectively collected as part of a large randomised controlled trial. The ground location of the injury was matched to the nearest corresponding ground hardness Clegg hammer readings, in gravities (g), which were classified from unacceptably low (<30 g) to unacceptably high hardness (>120 g). Results: Clegg hammer readings ranged from 25 to 301 g. Clegg hammer hardness categories from low/normal to high/normal were associated with the majority of injuries, with only 3.7% (13 injuries) on unacceptably high hardness and 0.3% (1 injury) on the unacceptably low hardness locations. Relative to the preferred range of hardness, the risk of sustaining an injury on low/normal hardness locations was 1.31 (95%CI: 1.06-1.62) times higher and 1.82 (95%CI: 1.17-2.85) times higher on locations with unacceptably high hardness. The more severe injuries occurred with low/normal ground hardness. Conclusions: Despite the low number of injuries, the risk of sustaining an injury on low/normal and unacceptably hard grounds was significantly greater than on the preferred range of hardness. It is a high-speed contact sport with a higher incidence of medically treated injuries when compared with most other organized sports. Hamstring injuries, ligament injuries to the knee or ankle, hip/groin injuries and tendinopathies are particularly common and often result in considerable time lost from sport. Consequently, the prevention of lower limb injuries is a priority for both community and elite Australian football organizations. There is considerable literature available on exercise programmes aimed at reducing lower limb injuries in Australian football and other running-related sports. The quality and outcomes of these studies have varied considerably, but indicate that exercise protocols may be an effective means of preventing lower limb injuries. Despite this, there has been limited high-quality and systematic evaluation of these data. Objective: The aim of this literature review is to systematically evaluate the evidence about the benefits of lower limb injury prevention exercise protocols aimed at reducing the most common severe lower limb injuries in Australian football. Methods: The Cochrane Central Register of Controlled Trials, the Cochrane Bone Joint and Muscle Trauma Group Specialized Register, MEDLINE and other electronic databases were searched, from January 1990 to December 2010. Papers reporting the results of randomized controlled trials (RCTs), quasi-RCTs, cohort and case-control studies were extracted. Primary outcomes were injury reduction or risk factor identification and/or modification. Secondary outcomes were adherence to any trialled interventions, injury severity and adverse effects such as secondary injuries and muscle soreness. The methodological quality of extracted manuscripts was assessed and results were collated. Results: Forty-seven papers were identified and reviewed of which 18 related to hamstring injury, eight related to knee or ankle ligament injury, five related to tendon injury and four were hip or groin injury related. Another 12 papers targeted general lower limb injuries. Most (n = 27 [57 %]) were observational studies, investigating injury risk factors. Twenty reported the results of intervention trials. Of these, 15 were efficacy trials reporting the effects of an intervention in reducing injury rates, four were biomechanical interventions in which the impact of the intervention on a known injury risk factor was assessed and one reported changes in injury risk factors as well as injury rates. The strength of the evidence base for exercise programmes for lower limb injury prevention was found to be limited, primarily due to the research methods employed, low adherence to interventions by the study participants and a lack of statistical power. Limited evidence obtained from a small number of RCTs suggests that balance and control exercises might be efficacious in preventing ankle ligament injuries and a programme involving a combination of balance and control exercises, eccentric hamstring, plyometrics and strength exercises could be efficacious in preventing all lower limb injuries. Conclusions: Overall, the evidence for exercise programmes as an efficacious lower limb injury prevention strategy is predominantly restricted to studies addressing injury aetiology and mechanisms. The findings of this review highlight the need to develop and test interventions in well designed population-based trials with an emphasis on promoting intervention uptake and adherence and, hence, intervention effectiveness. Using a qualitative multiple case study design, semi-structured interviews with community-AF coaches in Victoria, Australia, were conducted. Overall, coaches believed a range of strategies were important including: coach education, policy drivers, overcoming potential problem areas, a ‘try before you buy approach’, presenting empirical evidence and guidelines for injury-prevention exercise programs (IPEPs), forming strategic collaboration and working in partnership, communication and social marketing, public meetings, development of a coach hotline, and targeted multi-focused approaches. A shift to a culture whereby evidence-based IPEP practices in community-AF will take time, and persistent commitment by all involved in the sport is important. This will support the creation of strategies that will enhance the dissemination and scale-up of LL-IPEPs across community sport environments. The focus of research needs to continue to identify effective, holistic and multi-level interventions to support coaches in preventing LLIs. This could lead to the determination of successful strategies such as behavioural regulation strategies and emotional coping resources to implement LL-IPEPs into didactic curricula and practice. This paper presents a generalizable process for developing implementable sports injury prevention interventions, including a case study applying the process to develop a lower limb injury prevention exercise training program (FootyFirst) for community Australian football. Methods: The intervention development process is underpinned by 2 complementary premises: (1) that evidence-based practice integrates the best available scientific evidence with practitioner expertise and end user values and (2) that research evidence alone is insufficient to develop implementable interventions. Results: The generalizable 6-step intervention development process involves (1) compiling research evidence, clinical experience, and knowledge of the implementation context; (2) consulting with experts; (3) engaging with end users; (4) testing the intervention; (5) using theory; and (6) obtaining feedback from early implementers. Following each step, intervention content and presentation should be revised to ensure that the final intervention includes evidence-informed content that is likely to be adopted, properly implemented, and sustained over time by the targeted intervention deliverers. For FootyFirst, this process involved establishing a multidisciplinary intervention development group, conducting 2 targeted literature reviews, undertaking an online expert consensus process, conducting focus groups with program end users, testing the program multiple times in different contexts, and obtaining feedback from early implementers of the program. Conclusion: This systematic yet pragmatic and iterative intervention development process is potentially applicable to any injury prevention topic across all sports settings and levels. It will guide researchers wishing to undertake intervention development.

Coaches believed sprinting, eccentric strength training and proper periodisation were effective strategies to reduce HSI risk. There's a disparity between beliefs and practices when using GPS to monitor sprinting, however, all coaches reported regular exposure to sprint training across both pre and in-season. Overstriding (n = 9) and pelvic instability (n = 6) were identified as key flaws in running mechanics. Conclusions: This information can be used to improve training strategies, whilst these findings indicate further investigations into sprint training and running mechanics for HSI risk reduction. © 2020 Elsevier Ltd

The role of sprint training in hamstring strain injury prevention for field sport athletes

- Description: Hamstring strain injuries (HSI) are regularly reported as the most common injury in many field-based sports with a high-speed running (HSR) component. Typically, these injuries occur during sprinting efforts, predominantly in the Biceps Femoris Long Head (BFLH). Furthermore, the burden of HSI is amongst the highest in field sports, and the risk of recurrent injury is elevated, and inflated by age. It is of interest to managers, technical coaches, physical preparation staff and athletes to reduce the risk of sustaining a HSI. Therefore, extensive research in HSI has identified several non-modifiable and modifiable risk factors, such as eccentric hamstring strength, BFLH fascicle length, and HSR exposure. Currently, almost all interventions designed to reduce the risk of HSI have utilised resistance training or stretching interventions. This is despite the scientific literature indicating that; 1) sprinting is the most common mechanism associated with hamstring strain injury, and 2) sprinting places a far greater demand on the hamstrings than interventions that are resistance or flexibility based. Therefore, the primary aim of this thesis was to establish the role of sprint training in HSI prevention in field sport athletes. The aim of Study 1 was to understand the beliefs and practices of professional Australian Football (AF) physical performance coaches towards the training and assessment prescribed to mitigate the risk of HSI. A Delphi-validated mixed methods survey was implemented to assess the beliefs and practices of High-Performance Managers of professional AF teams. All the participants in this study reported that they believed sprinting (acceleration and maximum speed) was the most common activity associated with HSI. Similarly, all participants also indicated they performed sprint training for injury prevention purposes. However, a range of relative speeds were reported to quantify HSR, whilst a sprint was reported as 85%Vmax. Whilst respondents reported they believed that sprint training was important, one participant indicated that they felt the need to be conservative with the prescription of sprint training for fear of injury, and the consequences associated with this. Due to the identified discrepancies in speed thresholds used in practical setting (Study 1) and academic settings (Literature Review), Study 2 was designed to determine the absolute, and relative speeds associated with jogging, running, striding, near maximum sprinting, and sprinting. The secondary aim was to visually describe the gait patterns associated with HSR and sprinting. Fifteen participants completed two data collection periods, where they performed a series of run throughs at different gait patterns. These gait patterns were adapted from a review of time-motion research in field sports. The running gaits of jogging, running, striding, near-maximum sprinting, and sprinting correspond with the relative speeds of 56%Vmax, 66%Vmax, 78%Vmax, 87%Vmax, and 100%Vmax, respectively. Significant (p = 0.01) differences were observed for all variables between striding, near-maximum sprinting, and sprinting. This highlights that previously reported thresholds in Study 1 are likely not quantifying the true sprinting demands. Publicly available injury information indicates a spike in HSI during the 2018 Australian Football League (AFL) season, a trend that occurred in the relatively uninterrupted 2020 AFL season. As Study 2 indicated that thresholds used to quantify the demands of sprinting in training and competition, Study 3 primarily aimed to determine the amount of running completed in relative speed bands during the pre-season and then the first eight weeks of the season. The secondary aim was to determine how individual athletes vary from the group average across the pre-season and the first eight rounds of the season. This study completed a retrospective analysis of 55 professional Australian footballers during the pre-season and first eight rounds of the 2019 season. Significantly less (p < 0.05) weekly volume was reported during the first 8 weeks of the season in the speed thresholds of 71-80%Vmax, 81-90%Vmax, and 91-100%Vmax. However, athletes in this study only completed an average of 18m above 90%Vmax in the pre-season, a stimulus too low to achieve an injury prevention and conditioning effect. Individual analysis highlighted that whilst a small number of athletes achieved a consistent sprinting load, several athletes did not achieve more than 2m above 90%Vmax during both the pre and in-season periods. This lack of preparation may provide a rationale for the injury rates during the first eight rounds. To improve the preparation of athletes using a sprinting stimulus, Study 4 focussed on addressing modifiable risk factors for HSI. The primary aim of this study examined the effects of acceleration and maximum speed sprint training of eccentric hamstring strength, BFLH fascicle length, and sprint performance. Initially, a training study with 60 participants was planned, but due to the Victorian Government’s COVID-19 restrictions, this was modified to a case study design with 11 participants. A 6-week training intervention consisting of acceleration sprinting (<15m) or maximum speed sprinting with a gradual build up (flying 10-20m) was implemented. Participants that performed acceleration or maximum speed sprinting improved Biceps Femoris fascicle length by 23% and 20%, respectively. This was an increase of more than the Minimum Detectable Change (MDC95 = 0.96cm, 95% CI =0.93 – 0.99cm). The participants who completed normal training experienced no change in the BFLH fascicle length. Neither sprint training intervention produced a change in eccentric hamstring strength that was greater than the typical error in the test. Both sprinting interventions improved sprint performance outcomes, however both acceleration and maximum speed training produced a larger improvement in maximum speed (-11% and -9%, respectively) than in acceleration qualities (-2% and -3%, respectively). Both acceleration and maximum speed training incurred positive improvement in relative force production (F(0) (N/kg)) and relative power production (Pmax (W/kg)). This was similar for DRF and RFpeak and indicates that sprint training without resistance will improve Force-Velocity-Power Variables. This project has provided new information regarding the role of sprint training for hamstring strain injury prevention. Firstly, both 6 weeks of low volume acceleration and maximum speed sprinting improve BFLH fascicle length. Secondly, currently employed velocity thresholds in professional AF are likely too slow to quantify the HSR and sprinting demands. Thirdly, analysis using thresholds that are faster than previously reported revealed that the overall volume of sprinting (>90%Vmax) is too low to elicit an injury prevention or training stimulus. This information will inform the future practices of key stakeholders surrounding sprint training in field sport athletes. The identification of the positive benefits of maximum speed sprinting, and a faster, more accurate definition of sprinting has large potential to improve hamstring strain injury prevention and increase physical performance.
