- Description: BACKGROUND: Concussion is a serious problem in many contact sports, including rugby union football. The study's primary aim was to measure the efficacy of padded headgear in reducing the rates of head injury or concussion. METHODS: A cluster randomized controlled trial with three arms was conducted with rugby union football teams as the unit of randomization. Teams consisted of males participating in under 13-, 15-, 18-, and 20-yr age group competitions. The interventions were "standard" and "modified" padded headgear. Headgear wearing and injury were measured for each study team at each game over two seasons. RESULTS: Eighty-two teams participated in year 1 and 87 in year 2. A total of 1493 participants (10,040 player hours) were in the control group, 1128 participants (8170 player hours) were assigned to the standard headgear group, and 1474 participants (10,650 player hours) were assigned to the modified headgear group. The compliance rates were low in all groups, but 46% of participants wore standard headgear. An intention-to-treat analysis showed no differences in the rates of head injury or concussion between controls and headgear arms. Incidence rate ratios for standard headgear wearers referenced to controls were 0.95 and 1.02 for game and missed game injuries. Analyses of injury rates based on observed wearing patterns also showed no significant differences. Incidence rate ratios for standard headgear wearers referenced to nonwearers were 1.11 and 1.10 for game and missed game injuries. CONCLUSIONS: Padded headgear does not reduce the rate of head injury or concussion. The low compliance rates are a limitation. Although individuals may choose to wear padded headgear, the routine or mandatory use of protective headgear cannot be recommended. © 2009 by the American College of Sports Medicine.

- Description: Agility skill is important in Australian Rules football (ARF) as it enables an attacking player to successfully evade an opponent. To date, no research has examined offensive agility techniques in ARF. There were two purposes of this study: first, to compare the change of direction (COD) speed of three offensive agility techniques, and second, compare the reaction speed and accuracy when observing the same techniques from a defensive perspective. The techniques included the side-step, shuffle, and split-step. Seventeen players from an Australian Football League club were required to perform four trials of each technique. COD speed was expressed as a total time, and divided into entry, foot plant preparation, and exit time. In addition, nineteen players from the same club were assessed on a video-based reaction test. Players were required to respond by depressing a thumb switch to indicate whether the player on screen changed direction to the left or right. From an offensive perspective, the split-step foot plant preparation time (0.66. s) was significantly slower than both the side-step (0.7 s) and shuffle (0.75 s) (p< 0.05), but there were no significant differences in entry and exit times between techniques. From a defensive perspective, the players were significantly slower and less accurate when reacting to the player in the video performing the split-step (0.19. s) and shuffle (0.15. s) compared to the side-step (0.12 s) (p< 0.05). In a one-on-one situation in ARF, the split-step may be the most effective offensive technique. Not only was it slower and less accurate to react to, the exit speed following the change in direction was not significantly slower than the side-step. However, a performer would need to consider the cost of a slower foot plant preparation time versus the potential to deceive an opponent when under time stress. © 2010 Sports Medicine Australia.

- Description: Purpose: The purpose of this study was to determine the relationships between split times within sprint tests over 30 m and 40 m in elite Australian Rules footballers. Methods: Data were analyzed from two Australian Football League (AFL) clubs. The first club (n = 35) conducted a 40-m sprint test and recorded split times at 10 m and 20 m. The second club (n = 30) conducted a 30-m sprint test and recorded splits at 10 m and 20 m. Analyses included calculation of Pearson correlations and common variances between all the split times as well as "flying" times (20-40 m for the first club and 20 to 30 m for the second club). Results: There was a high correlation (r = 0.94) between 10-m time and 20-m time within each club, indicating these measures assessed very similar speed qualities. The correlations between 10-m time and times to 30 m and 40 m decreased, but still produced common variances of 79% and 66% respectively. However when the "flying" times (20-40 m and 20-30 m) were correlated to 10-m time, the common variances decreased substantially to 25% and 42% respectively, indicating uniqueness. Conclusions: It was concluded that 10-m time is a good reflection, of acceleration, capabilities and either 20 to 40 m in a 40-m sprint test or 20 to 30 m in a 30-m sprint test can be used to estimate maximum speed capabilities. It was suggested that sprint tests over 30 m or 40 m can be conducted indoors to provide useful information about independent speed qualities in athletes. © 2008 Human Kinetics, Inc.

- Description: Objectives: To obtain benefits from sports injury prevention programs, players are instructed to perform the exercises as prescribed. We developed an observational checklist to measure the quality of exercise performance by players participating in FootyFirst, a coach-led, exercise-based, lower-limb injury prevention program in community Australian Football (AF). Design: Observational. Methods: The essential performance criteria for each FootyFirst exercise were described in terms of the technique, volume and intensity required to perform each exercise. An observational checklist was developed to evaluate each criterion through direct visual observation of players at training. The checklist was trialled by two independent raters who observed the same 70 players completing the exercises at eight clubs. Agreement between observers was assessed by Kappa-statistics. Exercise fidelity was defined as the proportion of observed players who performed all aspects of their exercises correctly. Results: The raters agreed on 61/70 observations (87%) (Kappa = 0.72, 95% CI: 0.55; 0.89). Of the observations with agreed ratings, 41 (67%) players were judged as performing the exercises as prescribed. Conclusions: The observational checklist demonstrated high inter-rater reliability. Many players observed did not perform the exercises as prescribed, raising concern as to whether they would be receiving anticipated program benefits. Where quality of exercise performance is important, evaluation and reporting of program fidelity should include direct observations of participants.

- Description: The aim of this study was to verify if yo-yo intermittent recovery test (level 2) (yo-yo IR2) score is linked to Australian football (AF) performance through match exercise intensity. Six week prospective study design. Twenty-one data sets were recorded from nine individual players that completed the yo-yo IR2, and played an Australian Football League match in the first five rounds of the 2010 season wearing a global positioning system (GPS) unit. Simple mediation modelling was used to analyse the inter-relationship between yo-yo IR2 score, match exercise intensity and AF performance. Playing position and experience were also incorporated into the model to identify conditional affects. A significant direct relationship was observed between yo-yo IR2 and number of ball disposals (p<0.1) and a significant indirect relationship was observed between yo-yo IR2 and number of ball disposals through distance travelled at high intensity (HIR mmin-1) (p<0.1). Moderation analysis showed that playing position affected the relationship between of yo-yo IR2 and HIR mmin-1 (p<0.1) and HIR mmin-1 and total ball disposals (p<0.1). Playing experience also significantly affected the relationship between HIR mmin-1 and total ball disposals. This study is the first to identify the effects of yo-yo IR2 on total ball disposals through HIR mmin-1 performed during AF matches, and that playing position and playing experience affect these interactions. © 2011 Sports Medicine Australia.

- Description: Are the perceptual and decision-making components of agility trainable? A preliminary investigation. J Strength Cond Res 25(5): 1240-1248, 2011-Agility is an open motor skill; requiring change of direction speed (CODS) and perceptual and decision-making ability. The aim of this study was to determine whether the perceptual and decision-making component of agility can be trained. Fifteen rugby league players were tested on a sport-specific reactive agility test (RAT) and a CODS test. Players were then allocated to a training group (n = 8) or a nontraining group (n = 7). The training group underwent 3 weeks of reactive agility training that was designed to enhance perceptual and decision-making ability. After 3 weeks, all players were tested again. The training group's mean reactive agility time was 1.92 +/- 0.17 seconds preintervention and 1.66 +/- 0.14 seconds postintervention. The nontraining group's mean reactive agility time was 1.89 +/- 0.16 and 1.87 +/- 0.15 seconds, respectively. Mean CODS time for the training group was 1.64 +/- 0.15 seconds preintervention and 1.66 +/- 0.14 seconds postintervention. The nontraining group's mean CODS time was 1.61 +/- 0.12 and 1.62 +/- 0.12 seconds. Mean perception and response time for the training group, measured on the RAT, was 0.33 +/- 0.33 seconds preintervention and 0.04 +/- 0.22 seconds postintervention. The nontraining group's values were 0.34 +/- 0.20 and 0.27 +/- 0.28 seconds, respectively (results are +/-sigma). Differences in mean reactive agility time and perception and response time from pre to postintervention for the training group were statistically significant, as were differences in those values between the training and nontraining group post intervention. All other comparisons were not. Results from this study suggest that the perceptual and decision-making components of agility are trainable. Coaches should incorporate some open motor skills training in their programs when training agility.
'In a blink of an eye your life can change ' : Experiences of players sustaining a rugby-related acute spinal cord injury

- Description: Background Though rare, rugby union carries a risk for serious injuries such as acute spinal cord injuries (ASCI), which may result in permanent disability. Various studies have investigated injury mechanisms, prevention programmes and immediate medical management of these injuries. However, relatively scant attention has been placed on the player's experience of such an injury and the importance of context. Aim The aim of this study was to explore the injury experience and its related context, as perceived by the catastrophically injured player. Methods A qualitative approach was followed to explore the immediate, postevent injury experience. Semi-structured interviews were conducted with 48 (n=48) players who had sustained a rugby-related ASCI. Results Four themes were derived from the data. Participants described the context around the injury incident, which may be valuable to help understand the mechanism of injury and potentially minimise risk. Participants also described certain contributing factors to their injury, which included descriptions of foul play and aggression, unaccustomed playing positions, pressure to perform and unpreparedness. The physical experience included signs and symptoms of ASCI that is important to recognise by first aiders, fellow teammates, coaches and referees. Lastly, participants described the emotional experience which has implications for all ASCI first responders. Significance All rugby stakeholders, including players, first responders, coaches and referees, may gain valuable information from the experiences of players who have sustained these injuries. This information is also relevant for rugby safety initiatives in shaping education and awareness interventions.

- Description: Objectives: Sporting bodies have developed guidelines for managing community-level players with suspected concussion in response to international consensus statements on concussion in sport. The purpose of this study was to examine the factors that influence the intended use of concussion guidelines among community-level coaches and sports trainers from two popular football codes in Australia: Australian football and rugby league. Design: Cross-sectional survey. Methods: The survey, based on an extended theory of planned behaviour model, was completed by 183 Australian football coaches, 121 Australian football sports trainers, 171 rugby league coaches, and 142 rugby league sports trainers. Results: Personal norms and self-efficacy were significant predictors of intention to use concussion guidelines, although the relationship between self-efficacy and intention was stronger among Australian football coaches than rugby league coaches. Analysis of the salient beliefs that underpin self-efficacy found that coaches, irrespective of football code, felt less familiar (2 = 25.70, p < 0.001) and less experienced (2 = 31.56, p < 0.001) than sports trainers in using the concussion guidelines. At the same time, Australian football personnel, irrespective oftheir team role, feltthatthey had insufficienttime (2 = 8.04, p < 0.01) and resources (2 = 12.31, p < 0.001) to implement the concussion guidelines relative to rugby league personnel. Conclusions: Programmes aimed at increasing the intended use of sport concussion guidelines should focus on enhancing self-efficacy and leveraging personal norms. Increasing coaches’ familiarity and experience in using the concussion guidelines would also be warranted, as would finding ways to overcome the perceived time and resource constraints identified among Australian football personnel.

- Description: Objectives: To investigate changes from baseline on SCAT3 as a result of football game exposure, and association with X2 Patch measured head acceleration events in amateur Australian footballers. Design: Prospective cohort. Methods: Peak linear acceleration (PLA) of the head (>10 g) was measured by wearable head acceleration sensor X2 Biosystems X-Patch in male (n = 34) and female (n = 19) Australian footballers. SCAT3 was administered at baseline (B) and post-game (PG). Results: 1394 head acceleration events (HEA) >10 g were measured. Mean and median HEA PLA were recorded as 15.2 g (SD = 9.2, range = 10.0–115.8) and 12.4 g (IQR = 11.0–15.6) respectively. No significant difference in median HEA PLA (g) was detected across gender (p = 0.55), however, more HEAs were recorded in males (p = 0.03). A greater number (p = 0.004) and severity (p < 0.001) of symptoms were reported PG than at B. No significant association between number of HEA or median PLA, and SCAT3 change scores (p > 0.05 for all), was identified for either gender. Conclusions: Increase in symptom severity post game was not associated with X2 measured HEA. Males sustained more HEA, however HEA PLA magnitude did not differ across gender. Further work on the validation of head acceleration sensors is required and their role in sports concussion research and medical management. © 2017 Sports Medicine Australia
Igniting the pressure acclimatization training debate: Contradictory pilot-study evidence from Australian football

- Description: In Australian Football, set shot goal kicking is when a player has a shot at goal after taking a mark (i.e., catching a kicked ball) or receiving a free kick (i.e., penalty from an opposing player). In the past two decades, Australian football has improved in nearly all aspects of the game, yet set shot goal kicking accuracy has declined. The purpose of the current pilot study was to investigate whether pressure acclimatization training improves Australian football goal kicking under pressure. Twelve football players assigned to either an experimental (EG) or a control group (CG) completed a pre-intervention test, intervention, and post-intervention test. During the pre-intervention and post-intervention tests, participants attempted 10 goal kicking trials under low pressure (LP) and high pressure (HP) conditions. After five attempts, participants completed an anxiety questionnaire. During the intervention period, the EG practiced under HP, while the CG practiced under LP. These pilot results indicated a significant increase in participants ' anxiety from LP to HP for both groups and a significant decrease in accuracy from pre-intervention to post-intervention. In the post-intervention test, the CG was more accurate under HP than the EG, indicating no significant advantage by practicing under pressure and contradicting other acclimatization studies. These results may ignite a debate about the benefits of acclimatization training, but caution should be exercised when interpreting the results considering the pilot nature of the study. Using video analysis for concussion surveillance in Australian football

- Description: Objectives: The objectives of the study were to assess the relationship between various player and game factors and risk of concussion; and to assess the reliability of video analysis for mechanistic assessment of concussion in Australian football. Methods: All impacts and collisions resulting in concussion were identified during the 2011 Australian Football League season. An extensive list of factors for assessment was created based upon previous analysis of concussion in Australian Football League and expert opinions. The authors independently reviewed the video clips and correlation for each factor was examined. Results: A total of 82 concussions were reported in 194 games (rate: 8.7 concussions per 1000 match hours; 95% confidence interval: 6.9-10.5). Player demographics and game variables such as venue, timing of the game (day, night or twilight), quarter, travel status (home or interstate) or score margin did not demonstrate a significant relationship with risk of concussion; although a higher percentage of concussions occurred in the first 5 min of game time of the quarter (36.6%), when compared to the last 5 min (20.7%). Variables with good inter-rater agreement included position on the ground, circumstances of the injury and cause of the impact. The remainder of the variables assessed had fair-poor inter-rater agreement. Common problems included insufficient or poor quality video and interpretation issues related to the definitions used. Conclusions: Clear definitions and good quality video from multiple camera angles are required to improve the utility of video analysis for concussion surveillance in Australian football. - Description: Objectives: Video analysis can provide critical information to improve diagnostic accuracy and speed of clinical decision-making in potential cases of concussion. The objective of this study was to validate a hierarchical flowchart for the assessment of video signs of concussion, and to determine whether its implementation could improve the process of game day video assessment. Methods: All impacts and collisions potentially resulting in a concussion were identified during 2012 and 2013 Australian Football League (AFL) seasons. Consensus definitions were developed for clinical signs associated with concussion. A hierarchical flowchart was developed based on the reliability and validity of the video signs of concussion. Ninety videos were assessed, with 45 incidents of clinically confirmed concussion, and 45 cases where no concussion was sustained. Each video was examined using the hierarchical flowchart, and a single response was given for each video based on the highest-ranking element in the flowchart. Results: No protective action, impact seizure, motor incoordination or blank/vacant look were the highest ranked video signs in almost half of the clinically confirmed concussions, but in only 8.8% of non-concussed individuals. The presence of facial injury, clutching at the head and slow to get up were the highest ranked sign in 77.7% of non-concussed individuals. Conclusions: This study suggests that the implementation of a flowchart model could improve timely assessment of concussion, and it identifies the video signs that should trigger automatic removal from play. Quantification of Achilles and patellar tendon structure on imaging does not enhance ability to predict self-reported symptoms beyond grey-scale ultrasound and previous history

- Description: Background: Tendon pathology on imaging has been associated with an increased risk of developing symptoms. This evidence is based on classifying the tendon as normal or pathological. It is unclear whether the extent of tendon pathology is associated with the development or severity of symptoms. Objectives: To investigate whether the presence and extent of tendon pathology on ultrasound tissue characterisation (UTC), or a previous history of symptoms, were associated with the development of symptoms over a football season. Methods: 179 male Australian football players underwent UTC imaging of their Achilles and/or patellar tendon at the start of the pre-season. Players completed monthly OSTRC overuse questionnaires to quantify the presence and severity of Achilles and/or patellar tendon symptoms. Risk factor analysis was performed to identify associations between imaging and the development of symptoms. Results: A pathological Achilles tendon increased the risk of developing symptoms (RR = 3.2, 95%CI 1.7–5.9). Conversely, a pathological patellar tendon was not significantly associated with the development of symptoms (RR = 1.8, 95%CI 0.9–3.7). Quantification of tendon structure using UTC did not enhance the ability to identify athletes who developed symptoms. Previous history of symptoms was the strongest predictor for the development of symptoms (Achilles RR = 3.0 95%CI 1.8–4.8; patellar RR = 3.7 95%CI 2.2–6.1). Conclusion: Tendon pathology was associated with the development of self-reported symptoms; however previous history of symptoms was a stronger risk factor. The extent of disorganisation quantified by UTC should not be used as a marker for the presence or severity of current and future symptoms.

- Description: Shared use of grounds allowed Australian cricket and football to subsidize each other, but cartel arrangements that determined the use of stadiums and the distribution of benefits and costs between sports may have been less than optimal. Estimation of deadweight losses from the use of stadiums is not possible in the absence of a counterfactual specifying the level of demand if the behaviour of cartel members had been coordinated more effectively. Archival, financial and attendance report data can be used to estimate increases in actual demand under alternative scenarios. In Melbourne and Adelaide, the controlling bodies of cricket and football uncured significant losses in welfare from joint use of their cities’ major stadium, due to the importance they attached to non-monetary aspects of utility.

Injury surveillance is therefore required to inform the development and implementation of strategies to increase safety in professional football codes. EVIDENCE ACQUISITION: A systematic search of peer reviewed and available grey literature was conducted to identify publications presenting methodological details of ongoing injury surveillance systems in professional football codes. Data extracted included population under surveillance, player/team coverage, injury definition and other relevant information. EVIDENCE SYNTHESIS: Following literature search and review, seven injury surveillance systems were identified that record ongoing data within professional football codes. A range of methodologies was used across the surveillance systems with variations in the definitions of injury, who records the data, the data-recording tool and dissemination of the data. CONCLUSIONS: There are many benefits for football associations, clubs and players themselves in conducting comprehensive injury surveillance. A range of surveillance methods is employed across the different football codes including injury definitions. Some professional football codes have developed consensus statements, which provide guidance on key issues relating to injury surveillance. Dissemination of findings differed with some systems publishing via peer review journals, with others through annual disclosure to the media. Financial considerations and funding are a crucial factor for creating and maintaining an injury surveillance system. Findings from professional football injury surveillance have been used to implement rule changes and develop preventive programmes, all aimed at increasing player safety.

- Description: Objectives: The field of sport injury prevention has seen a marked increase in published research in recent years, with concomitant proliferation of lay sport safety resources, such as policies, fact sheets and posters. The aim of this study was to catalogue and categorise the number, type and topic focus of sport safety resources from a representative set of key organisations. Design: Cataloguing and qualitative document analysis of resources available from the websites of six stakeholder organisations in Australia. Setting: This study was part of a larger investigation, the National Guidance for Australian Football Partnerships and Safety (NoGAPS) project. Participants: The NoGAPS study provided the context for a purposive sampling of six organisations involved in the promotion of safety in Australian football. These partners are recognised as being highly representative of organisations at national and state level that reflect similarly in their goals around sport safety promotion in Australia. Results: The catalogue comprised 284 resources. More of the practical and less prescriptive types of resources, such as fact sheets, than formal policies were found. Resources for the prevention of physical injuries were the predominant sport safety issue addressed, with risk management, environmental issues and social behaviours comprising other categories. Duplication of resources for specific safety issues, within and across organisations, was found. Conclusions: People working within sport settings have access to a proliferation of resources, which creates a potential rivalry for sourcing of injury prevention information. Important issues that are likely to influence the uptake of safety advice by the general sporting public include the sheer number of resources available, and the overlap and duplication of resources addressing the same issues. The existence of a large number of resources from reputable organisations does not mean that they are necessarily evidence based, fully up to date or even effective in supporting sport safety behaviour change. © 2016, BMJ Publishing Group. - Description: Injuries are a major contributor to healthcare costs and individuals' health and disability status. In response to the overall public health burden, injuries were one of the first medical conditions identified as an Australian National Health Priority Area. Our previous epidemiological research has shown that sports injuries, especially those sustained through formal and highly competitive sport, are often associated with considerable pain and dysfunction. They have significant ongoing impact on quality of life and need for medical treatment, including in the hospital setting.They are also a major barrier towards both the uptake and continuance of health-generating physical activity guidance.
The inter-tester reliability of the squeeze and bent-knee-fall-out tests in elite academy football players

- Description: Objectives: To evaluate the inter-tester reliability of two common tests for assessing hip and groin injuries, using a protocol closely reflecting normal clinical conditions. Design: Same-session, single-trial, inter-tester reliability study. Setting: Elite European football academy. Participants: Two testers and fifty-eight football players aged 14–21 years. Main outcome measures: Inter-tester reliability of the squeeze (in approximately 45° hip flexion) and bent-knee-fall-out (BKFO) tests, evaluated with intraclass correlation coefficients (two-way mixed, single measure) and minimal detectable change. Results: Both tests demonstrated excellent inter-tester reliability. The intraclass correlation coefficients were 0.92 (95% CI: 0.87–0.95) for the squeeze test, 0.91 (95% CI: 0.67–0.96) for the BKFO test on the right side and 0.92 (95% CI 0.87–0.95) for BKFO on the left side. The minimal detectable changes were 7.21 mmHg for the squeeze test, 0.95 cm and 0.75 cm for BKFO (right) and BKFO (left) respectively. Conclusion: The squeeze and the BKFO tests demonstrated excellent inter-tester reliability in elite academy football players, using a protocol reflective of normal clinical conditions.
Online news media reporting of football-related fatalities in Australia : A matter of life and death

- Description: Objectives: While deaths in sports settings are rare, they do occur. To develop an understanding of the sports and people most at risk, and to identify opportunities for prevention, routine and systematic data detailing the occurrence of these fatalities is required. There is currently no routine reporting of data of this nature in Australia. As there is often strong community interest in these incidents, the media offers an opportunity for surveillance. However before this can occur, understanding of the terminology used by the media is required. The aim of this study was to identify the terminology most frequently used in online Australian news media coverage of football-related deaths. Design: Retrospective review of online news media. Methods: Three databases were searched for online news media reports of people who died while participating in football (all football codes) in Australia. A descriptive analysis of terminology was undertaken to identify the common language applied. Results: Thirty-four football-related fatalities in Australia were identified between 2010-2016, via 149 separate articles. The most frequent terms identified in the media items were: Family; Club; Rugby; Football; Player; League; Died; Game; Death; Life; Loved; Hospital; Match; Young; Community; Playing; Friends; Sport; Heart; AFL [Australian Football League]. Conclusions: This study identified terminology used in reporting football-related fatalities in Australia, identifying common reference to terms relating to 'death' as metaphors and the frequent celebration of 'life.' The findings suggest that a reliance on researcher-generated terminology will be insufficient to reflect media discourse in prospective monitoring of sports deaths for surveillance.
