Injury epidemiology among Australian female cricketers

- Description: Cricket is a male-dominated sport; however, its popularity among females is increasing. Like other sports, participation in cricket poses the risk of injury to players. Injury problems for female cricketers are virtually unknown, as studies examining cricket injuries include almost exclusively male participants. In other sports, the types of injuries experienced by men and women are known to be different. Therefore, a clear understanding of the extent and types of injuries sustained by female cricket players is required, to underpin appropriately targeted injury prevention strategies. This thesis provides the first detailed epidemiological profile of cricket injuries sustained by women, by: 1. conducting a systematic review describing injuries in competitive team bat or stick sports in women, to enable cricket injuries to be viewed within the perspective of wider, but relevant, injury data, 2. evaluating existing data sources relating to hospital admissions from Victoria and Queensland and successful insurance claims across Australia, 3. examining the nature and incidence of cricket injuries in elite female players using Cricket Australia’s Athlete Management System, and 4. conducting a nationwide self-report survey of injuries during the 2014–15 season. This PhD research represents participants from different levels of play, across age groups and across Australia. The findings indicate that incidence of injuries for female cricketers were higher than expected based on previous findings in comparable sports, except when considered in relation to insurance claims. The cricket injury rate across hospital presentations, insurance claims, the AMS (Fair Play AMS 2016) and self-reported survey data, each of which represents different level of the sports injury pyramid, identified all-rounders and pace bowlers as having a higher incidence of injury than players in other positions. The highest frequency of reported injuries were in the head, hands, knees and ankles. The nature of the most common injuries were dislocations/sprains/strains, fractures, muscle injury, joint injury and gradual onset injuries. At the elite-level, lumber spine stress fractures accounted for a significant amount of time-loss from the sport. In this thesis, findings from the insurance claims, self-reported survey and AMS (Fair Play AMS 2016) data indicated that most injuries were of a low severity and were more likely to be treated outside of healthcare facilities such as hospitals. In summary, patterns of the most common injuries, in terms of anatomical location and nature of the injuries, were consistent throughout community-level players with some similarities to elite-level players. However, the injury mechanisms and risk factors may differ depending on the level of competition and player’s skill. Recommendations are that ongoing injury surveillance should be conducted at all levels of the sport, and surveillance methodology should be tailored to the specific setting, personnel and available resources. Therefore, before implementing an injury surveillance system at the community-level of the sport, more research is needed to fully understand what type of injury surveillance system might be feasible and suitable in this context.

- Description: Study design: Cross-sectional study. Objectives: To identify socio-demographic and injury-related factors that contribute to activity limitations and participation restrictions in people with spinal cord injury (SCI) in Bangladesh. Setting: Centre for the Rehabilitation of the Paralysed (CRP), Savar, Dhaka, Bangladesh. Methods: This study involved 120 (83% men) participants with SCI; their median (interquartile range) age and injury duration were 34 (25-43) years and 5 (2-10) years, respectively. Data were collected from the follow-up records kept by the Community Based Rehabilitation (CBR) unit of CRP and a subsequent home visit that included interview-Administered questions, questionnaires, and a neurological examination. The dependent variables were activity limitations and participation restrictions, assessed with the World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0, scored 0-100; a high score indicates greater activity limitations and participation restrictions). Independent variables included socio-demographic factors (i.e., age, sex, marital status, educational level, monthly household income, employment status, and place of residence) and injury-related factors (i.e., injury duration, cause of injury, injury severity, and type of paralysis). Multivariable linear regression analyses were performed to identify the factors that independently contributed to activity limitations and participation restrictions. Results: Three significant independent variables explained 20.7% of the variance in activity limitations and participation restrictions (WHODAS 2.0 score), in which tetraplegia was the strongest significant contributing factor, followed by rural residence and complete injury. Conclusions: This study would indicate that tetraplegia, complete injury, and residing in a rural area are the major contributions in limiting the activity and participation following SCI in Bangladesh.

- Description: Objectives: To present the first comprehensive epidemiological profile of hospital-treated injuries sustained by female cricketers from 2002-2003 to 2013-2014 in Victoria, Australia. Design: Analysis of routinely collected hospital data (detailed case-series). Methods: A retrospective analysis of hospital-treatment data associated with cricket injuries sustained by women between 1 July 2002 and 30 June 2014, inclusive were extracted from databases held by the Victorian Injury Surveillance Unit in Australia. Results: Over the 12-year period, 668 cases were treated in Victoria. Of these, 547 were emergency department (ED)-presentations. There were 121 hospital-admissions, of which, the length of stay was <2 days for 78.5% cases. All cases were treated and released, and no fatalities were reported. The 10-14 year age group most frequently presented to ED (19.9%) and were most commonly admitted to hospital (16.5% of the total admissions). Fractures were the most common cause of hospital-admissions (47.1%) but only accounted for 17.2% of the ED-presentations. Dislocations, sprains and strains, were the most common (36.4%) cause of ED-presentations. The head was the most commonly injured anatomical location (27.8% of ED-presentations and 28.1% of hospital-admissions), followed by the wrist and hand (27.8% ED-presentations and 17.4% hospital-admissions). Conclusions: These findings provide the first overview of the nature of injuries requiring hospital attendance in female cricketers, and a foundation to inform the development of targeted injury prevention programs for female cricketers. The incidence, prevalence, nature, severity and mechanisms of injury in elite female cricketers : A prospective cohort study

- Description: Objectives: Incidence, prevalence, nature, severity and mechanisms of injury in elite female cricketers over two seasons from March 2014 to March 2016, inclusive. Design: Prospective cohort study. Methods: Injury data collected via Cricket Australia's Athlete Management System on all elite female players over two seasons were analysed. Profiles of the nature, anatomical location and mechanism of injuries were presented according to dominant player position. Injury incidence rates were calculated based on match playing hours. Results: There were 600 medical-attention injuries; with 77.7% players reporting ≥1 injury. There were 79.5% acute injuries compared to gradual onset injuries. Of the all medical-attention injuries, 20.2% led to time-loss; 34.7% were match-time-loss injuries. Match injury incidence was 424.7 injuries/10,000 h for all injuries and 79.3 injuries/10,000 h for time-loss injuries. Of all the injuries, 31.8% were muscle injuries and 16.0% joint sprains. Wrist and hand (19.8%), lumbar spine (16.5%) and knee (14.9%) injuries were the most common time-loss injuries. Six players sustained lumber spine bone stress injury that resulted in the most days missed due to injury (average 110.5 days/injury). Conclusions: There is a need to focus on specific injuries in female cricket, including thigh, wrist/hand and knee injuries because of their frequency, and lumbar spine injuries because of their severity.

Socio-economic and nutritional determinants of low birth weight in India

- Description: Background: Low birth weight (LBW) is an important risk factor for childhood morbidity and mortality, consequently an important public health concern. Aim: This study aims to identify significant socio-economic and nutritional determinants associated with LBW in India. Materials and Methods: Data from 2005 to 2006 National Family Health Survey-3 (NFHS-3) of India was analyzed. A total of 20,946 women (15-49 years) who gave birth at least once 5 years preceding the NFHS-3 were included in this study. Infant's LBW (<2500 grams) as outcome variable was examined in association with all independent predictors as infant's sex, maternal household wealth status, caste, age, education, body mass index (BMI), stature, anemia level, parity, inter-pregnancy interval, antenatal care received, and living place. Results: Almost 20% of the infants were born with LBW. Mother's low education level, BMI <18.5, short stature (height <145 centimeters) and lack of antenatal visits (<4 visits) were significant predictors of LBW. Male gender has a protective effect against LBW. Conclusion: Maternal education, nutritional status and antenatal care received are key determinants that need to be addressed to reduce prevalence of LBW in India. Continue implementation of multifaceted health promotion interventions are needed to address these factors effectively.
