Changing rural general practitioner practice : Evaluating health assessment uptake

- Description: The Enhanced Primary Care Package aimed to improve health and quality of life through enhancing primary health care for those over 75 years, Aboriginal and Torres Strait Islanders over 55 years and those with chronic conditions and multidisciplinary needs. A multi-level approach was implemented to promote the package. West Vic Division of General Practice conducted the General Practice Education Support and Community Linkages implementation program for the package in Western Victoria and focused on health assessments for those over 75 years. This research investigated what factors affected change in rural general practitioner practice through surveying general practitioners and collecting Health Insurance Commission data prior to and following the program. Patients were also interviewed to determine acceptance of health assessments and nurse involvement. The program increased awareness, but had little effect on increasing uptake, as only 53% of general practitioners began undertaking health assessments. However the general practitioners indicated an increased frequency of health assessment prescription. Health Insurance Commission data suggested an immediate increase in the use of the assessments, however the rate fluctuated and then declined. Lack of sustained uptake of the program was not associated with remuneration, as 77% of general practitioners did not regard finance as a barrier. Respondents’ major barrier was time (40% pre-education, 73% post-education). This data reflected a rural environment where general practitioners face competing priorities, time constraints, workforce shortage and long consultation lists. A notable change did however occur from practice nurse employment as the involvement of a practice nurse generally resulted in patient satisfaction with the assessment. It was apparent that a complex multifaceted and longer-term view is needed to address factors which limit rural general practitioners’ ability to change. This needs to be addressed at the Commonwealth level and not in isolation in order to produce an integrated framework to enhance and promote, rather than demand change.

Tertiary student connectedness : Intervention influence on student connectedness as measured in health and academic behaviours of regional tertiary students

- Description: At a time when university student diversity is heightened and when national government regulations have shifted their quality focus from support improvements to student achievement, little is known about the mechanism of the latter. University efforts to support quality, caring interactions between students and staff, known as connectedness, are currently favoured, and this thesis examines connectedness, mood, emotional well-being and academic behaviours of commencing students at a regional Australian university. This is done through the lens of a first semester intervention, known as Tertiary Learning Communities (TLC). Drawing on existing connectedness research, across both school and university settings and guided by a whole-school framework, details of the establishment of a suitable working party, the development of survey, pilot and intervention activities, and monitoring the ability of a cross-campus intervention to influence student connectedness and behaviour markers, are highlighted to advance further understanding of the mechanics of connectedness in a university setting. The developed survey, which collected data from undergraduate students early in their first semester, effectively captured perceptions of connectedness across a broad range of sources in addition to their mood, emotional well-being and academic student behaviours. Paired sample-tests assessed connectedness changes, and chi square analysis assessed behaviour changes when comparing the experimental and control groups on two occasions. A single intervention aligned to a first year core unit to support academic and social interactions, was shown to be ineffective in enhancing student connectedness during the semester of the intervention or the semester following the intervention. However, the level of connectedness decreases measured across ‘personal’, ‘other students’ and ‘lecturers’ were indeed significant, as was the finding that connectedness decreases for intervention participants exceeded the decreases of the control group. Furthermore, mood and emotional well-being challenges and the slow emergence of academic behaviours were also revealed. These findings provided support for future inclusive student support initiatives, maintained the involvement of working party members and extended support beyond the first semester to across first year.

Creating safety : intersection of healthcare and police response to violence against immigrant south asian women in British Columbia : a service provision model

- Description: Background Intimate Partner Violence (IPV), is an internationally prevalent health and safety issue impacting women. Immigrant South Asian women (ISAW) in British Columbia (BC) remain vulnerable to serious injuries and homicides despite efforts of healthcare and police services. It is critical to improve these responses. Knowledge of the challenges to responses can inform and improve services and the well-being of ISAW. Aims This study aimed to explore the needs of ISAW and the challenges of healthcare and police responses, how these may inform responses and to compare the perspectives of survivors, responders and experts to improve the health and safety of ISAW. Methods Design: a two-phased, mixed methods study. Phase 1: a purposive sample of 4 focus groups of 22 ISAW survivors from four cities in BC. An additional focus group involved five subject matter experts. Also conducted were seven face-to-face semi-structured interviews with twelve key informants. Phase 2: a convenience sample of 128 provincial responders completed an online survey. Results The key emerging themes for survivors were the difficulties in identifying and disclosing IPV and the responses to IPV. Responders identified the gaps in services, culturally and linguistically inappropriate policies and uncoordinated and non-collaborative efforts. For the ISAW, perceptions, fears, and socio-cultural impediments hindered their disclosure of IPV. They required culturally and linguistically trauma informed responses, policies and practices to overcome multiple and complex challenges. Co-ordinated, collaborative systemic responses to improve the health and safety of ISAW were identified by all participants, including appropriate risk assessment. Conclusion This study adds to current knowledge by contributing to the understanding of IPV of ISAW from different perspectives: ISAW survivors, responders and experts. This knowledge offers potential improvements to services and policy developments to reduce the health and safety risks to IPV survivors.
