Bias in amputation research; impact of subjects missed from a prospective study

- Description: For research findings to be generalized, a sample must be representative of the actual population of interest. Lower limb amputation is most frequently performed in older patients with vascular disease, a population that is often under-represented in research. The aim of this study was to explore the impact of selection bias by comparing characteristics from a sample included in a prospective study of phantom pain with the actual population who underwent amputation. Only 27% of all potential patients were referred during the first year of the prospective study. The referred patients were 8 years younger (p<0.001) and less likely to have had amputation because of a vascular condition, diabetes or infection (p = 0.003) than those not referred. There was also a significant difference in one year survival between the groups; 67% of referred patients survived compared with just 40% of non-referred patients (p = 0.004). The biased population in the phantom pain study may have resulted in an underestimation of phantom pain in the original study and subsequent protective factors should be considered within the context of the younger population reported. Selection bias is common in amputation research, and research methods to minimize its impact must be given greater attention.

A cross-sectional cohort design was used. Informal caregivers completed the Demand and Difficulty subscales of the Caregiving Burden Scale; and the Mutuality, Preparedness, and Global Strain subscales of the Family Care Inventory. Chi-square tests and logistic regression were used to examine the relationships between caregiver and care recipient variables and preparedness for caregiving. Twenty-nine informal caregivers who reported data on themselves and people with a moderate to severe TBI were recruited (referred to as a dyad). Most caregivers were female (n = 21, 72%), lived with the care recipient (n = 20, 69%), and reported high levels of burden on both scales. While most caregivers (n = 21, 72%) felt “pretty well” or “very well” prepared for caregiving, they were least prepared to get help or information from the health system, and to deal with the stress of caregiving. No significant relationships or predictors for caregiver burden or preparedness were identified. While caregivers reported the provision of care as both highly difficult and demanding, further research is required to better understand the reasons for the variability in caregiver experience, and ultimately how to best prepare caregivers for this long-term role. © 2020 by the authors. Can optimum solar radiation exposure or supplemented vitamin D intake reduce the severity of COVID-19 symptoms?

- Description: The foremost mortality-causing symptom associated with COVID-19 is acute respiratory distress syndrome (ARDS). A significant correlation has been identified between the deficiency in vitamin D and the risk of developing ARDS. It has been suggested that if we can reduce or modify ARDS in COVID-19 patients, we may significantly reduce the severity of COVID-19 symptoms and associated mortality rates. The increased mortality of dark-skinned people, who have a reduced UV absorption capacity, may be consistent with diminished vitamin D status. The factors associated with COVID-19 mortality, such as old age, ethnicity, obesity, hypertension, cardiovascular diseases, and diabetes, are all found to be linked with vitamin D deficiency. Based on this review and as a precautionary measure, it is suggested that the adoption of appropriate and safe solar exposure and vitamin D enriched foods and supplements should be considered to reduce the possible severity of COVID-19 symptoms. Safe sun exposure is deemed beneficial globally, specifically in low and middle- income countries, as there is no cost involved. It is also noted that improved solar exposure and vitamin D levels can reduce the impact of other diseases as well, thus assisting in maintaining general human well-being. © 2021 by the authors. - Description: The present study explored the changes in tobacco use patterns during the COVID-19 pandemic and their correlates among older adults in Bangladesh. This cross-sectional study was conducted among 1032 older adults aged ≥60 years in Bangladesh through telephone interviews in October 2020. Participants’ characteristics and COVID-19-related information were gathered using a pretested semi-structured questionnaire. Participants were asked if they noted any change in their tobacco use patterns (smoking or smokeless tobacco) during the COVID-19 pandemic compared to pre-pandemic (6 months prior to the survey). Nearly half of the participants (45.6%) were current tobacco users, of whom 15.9% reported increased tobacco use during the COVID-19 pandemic and all others had no change in their tobacco use patterns. Tobacco use was significantly increased among the participants from rural areas, who had reduced communications during COVID-19 compared to pre-pandemic (OR = 2.76, 95%CI:1.51–5.03). Participants who were aged ≥70 years (OR = 0.33, 95% CI: 0.14–0.77), widowed (OR = 0.36, 95% CI: 0.13–1.00), had pre-existing, non-communi-cable, and/or chronic conditions (OR = 0.44, 95% CI:0.25–0.78), and felt themselves at the highest risk of COVID-19 (OR = 0.31, 95% CI: 0.15–0.62), had significantly lower odds of increased tobacco use. Policy makers and practitioners need to focus on strengthening awareness and raising initia-tives to avoid tobacco use during such a crisis period. © 2021 by the authors. - Description: A 36-km road built in 1961 between Ayers Rock (Uluru) and the Olgas (Kata Tjuta), Northern Territory, Australia had seriously deteriorated by the 1980s. A newly aligned road, which was ecologically located and avoid sensitive Aboriginal sites, was completed early in 1991. The old road was rehabilitated by deep ripping, filling with imported sand and topsoil and by grading logs and windrows1 over the new surface. This paper reports on the effectiveness of the rehabilitation technique used in reinstating vegetation over 8 years and considers whether this rehabilitation aim was met. Plant colonisation and succession were monitored on 15 paired plots, one in the rehabilitated road and the other in the adjacent undisturbed habitat (with four exceptions) and also on the windrows. Sites at eight creek crossings and those subject to fire and rabbit activity were also monitored. Different landscape units responded in different ways to the rehabilitation. Herbaceous species from imported sand plain fill and top soil eventually dominated the road where they were introduced and were likely to persist in most areas because of local recruitment. The revegetation of the road has stabilised the old road surface as observed by the reduced erosion although succession did not always approach the reference site communities because of a greater resemblance to the Simpson Land System. Over the 8 years of monitoring considerable changes in vegetation occurred and are probably continuing. Only the road sites in the Simpson Land System approached the condition of the adjacent undisturbed vegetation because the vegetation of the other two land systems became closer to that of the Simpson Land System rather than to that of the surrounding vegetation. It is recommended that the introduced Buffel grass, known to alter landscape-level processes by reducing native herbaceous species and increasing risk of high intensity wildfire, which is most evident at creek crossings, should be controlled. - Description: There has been considerable interest in composition of gut microbiota in recent years, leading to a better understanding of the role the gut microbiota plays in health and disease. Most studies have been limited in their geographical and socioeconomic diversity to high-income settings, and have been conducted using small sample sizes. To date, few analyses have been conducted in low-income settings, where a better understanding of the gut microbiome could lead to the greatest return in terms of health benefits. Here, we have used quantitative real-time polymerase chain reaction targeting dominant and sub-dominant groups of microorganisms associated with human gut microbiome in 115 people living a subsistence lifestyle in rural areas of Papua New Guinea. Quantification of Clostridium coccoides group, C. leptum subgroup, C. perfringens, Bacteroides fragilis group, Bifidobacterium, Atopobium cluster, Prevotella, Enterobacteriaceae, Enterococcus, Staphylococcus, and Lactobacillus spp. was conducted. Principle coordinates analysis (PCoA) revealed two dimensions with Prevotella, clostridia, Atopobium, Enterobacteriaceae, Enterococcus and Staphylococcus grouping in one dimension, while B. fragilis, Bifidobacterium and Lactobacillus grouping in the second dimension. Highland people had higher numbers of most groups of bacteria detected, and this is likely a key factor for the differences revealed by PCoA between highland and lowland study participants. Age and sex were not major determinants in microbial population composition. The study demonstrates a gut microbial composition with some similarities to those observed in other low-income settings where traditional diets are consumed, which have previously been suggested to favor energy extraction from a carbohydrate rich diet. © 2015 PLOS ONE.

Quantification of Clostridium coccoides group, C. leptum subgroup, C. perfringens, Bacteroides fragilis group, Bifidobacterium, Atopobium cluster, Prevotella, Enterobacteriaceae, Enterococcus, Staphylococcus, and Lactobacillus spp. was conducted. Principle coordinates analysis (PCoA) revealed two dimensions with Prevotella, clostridia, Atopobium, Enterobacteriaceae, Enterococcus and Staphylococcus grouping in one dimension, while B. fragilis, Bifidobacterium and Lactobacillus grouping in the second dimension. Highland people had higher numbers of most groups of bacteria detected, and this is likely a key factor for the differences revealed by PCoA between highland and lowland study participants. Age and sex were not major determinants in microbial population composition. The study demonstrates a gut microbial composition with some similarities to those observed in other low-income settings where traditional diets are consumed, which have previously been suggested to favor energy extraction from a carbohydrate rich diet. © 2015 PLOS ONE.

Cognitive distortions among older adult gamblers in an Asian context

Methods: Singapore residents (citizens or permanent residents) aged 60 years and above, who were current or past regular gamblers were included in the study. Participants were recruited using a combination of venue based approach, referrals from service providers as well as by snowball sampling. In all, 25 in-depth interviews were conducted with older adult gamblers. The six-step thematic network analysis methodology was adopted for data analysis. Results: The mean age of the participants was 66.2 years. The majority were male (n = 18), of Chinese ethnicity (n = 16), with a mean age of gambling initiation at 24.5 years. Among older adult gamblers, cognitive distortions emerged as a significant global theme comprising three organizing themes-illusion of control, probability control and interpretive control. The organizing themes comprised nine basic themes: perception of gambling as a skill, near miss, concept of luck, superstitious beliefs, entrapment, gambler's fallacy, chasing wins, chasing losses, and beliefs that wins are more than losses. Conclusions: Cognitive distortions were endorsed by all gamblers in the current study and were shown to play a role in both maintaining and escalating the gambling behaviour. While the surface characteristics of the distortions had a culture-specific appearance, the deeper characteristics of the distortions may in fact be more universal than previously thought. Future research must include longitudinal studies to understand causal relationships between cognitive distortions and gambling as well as the role of culture-specific distortions both in the maintenance and treatment of the disorder. © 2017 Subramaniam et al. - Description: In our daily lives, information concerning temperature is often provided by means of colour cues, with red typically being associated with warm/hot, and blue with cold. While such correspondences have been known about for many years, they have primarily been studied using subjective report measures. Here we examined this correspondence using two more objective response measures. First, we used the Implicit Association Test (IAT), a test designed to assess the strength of automatic associations between different concepts in a given individual. Second, we used a priming task that involved speeded target discrimination in order to assess whether priming colour or thermal information could invoke the crossmodal association. The results of the IAT confirmed that the association exists at the level of response selection, thus indicating that a participant's responses to colour or thermal stimuli are influenced by the colour-temperature correspondence. The results of the priming experiment revealed that priming a colour affected thermal discrimination reaction times (RTs), but thermal cues did not influence colour discrimination responses. These results may therefore provide important clues as to the level of processing at which such colour-temperature correspondences are represented.

- Description: Marine currents have been identified as a considerable renewable energy source. Therefore, in recent years, research on optimising tidal stream farm layouts in order to maximise power output has emerged. Traditionally, computational fluid dynamics (CFD) models are used to model power output, but their computational cost is prohibitive within an optimisation algorithm. This paper uses surrogate models in place of CFD simulations to optimise the layout of tidal stream farm layouts. Surrogates are functions which are designed to emulate the behaviour of other models with radically reduced computational expense. Two surrogate models are applied and compared: artificial neural network (ANN) and k-nearest neighbours regression (k-NN). We measure their suitability by four criteria: accuracy, efficiency, robustness and performance within an optimisation algorithm. The results reveal that the ANN surrogate is superior in every criteria to the k-NN surrogate. However, the k-NN surrogate is also able to perform adequate optimisation. Finally, we demonstrate that optimisation relying solely on surrogate models is a viable approach, with dramatically reduced computational expense of optimisation. © 2016 The Authors. This study used independent clusters confirmatory factor analysis (ICM-CFA) and exploratory structural equation modeling (ESEM) to examine the scale’s factor structure. A general community sample of 203 (men = 43, women = 160) between 17 and 63 years completed the QCAE. Although both the five-factor oblique and second order factor models showed good model fit, and clarity in the pattern of factor loadings, in the second-order factor model, none of the primary factors loaded significantly on their respective secondary factors, thereby favoring the five-factor oblique model. The factors in this model were supported in terms of external validity. Despite this, the factor for PRO in this model showed low reliability for meaning interpretation. A revised four-factor oblique model without the PRO factor showed good fit, clarity in the pattern of factor loadings, and reliability and validity for the factors in this model, thereby suggesting this to be the best model to represent ratings on the QCAE. The implications of the findings for use and revision of the DASS-21 are discussed.

- Description: Unicellular diazotrophic cyanobacteria such as Cyanothece sp. ATCC 51142 (henceforth Cyanothece), temporally separate the oxygen sensitive nitrogen fixation from oxygen evolving photosynthesis not only under diurnal cycles (LD) but also in continuous light (LL). However, recent reports demonstrate that the oscillations in LL occur with a shorter cycle time of ∼11 h. We find that indeed, majority of the genes oscillate in LL with this cycle time. Genes that are upregulated at a particular time of day under diurnal cycle also get upregulated at an equivalent metabolic phase under LL suggesting tight coupling of various cellular events with each other and with the cell's metabolic status. A number of metabolic processes get upregulated in a coordinated fashion during the respiratory phase under LL including glycogen degradation, glycolysis, oxidative pentose phosphate pathway, and tricarboxylic acid cycle. These precede nitrogen fixation apparently to ensure sufficient energy and anoxic environment needed for the nitrogenase enzyme. Photosynthetic phase sees upregulation of photosystem II, carbonate transport, carbon concentrating mechanism, RuBisCO, glycogen synthesis and light harvesting antenna pigment biosynthesis. In Synechococcus elongates PCC 7942, a non-nitrogen fixing cyanobacteria, expression of a relatively smaller fraction of genes oscillates under LL condition with the major periodicity being 24 h. In contrast, the entire cellular machinery of Cyanothece orchestrates coordinated oscillation in anticipation of the ensuing metabolic phase in both LD and LL. - Description: In recent years, the widespread deployment of the Internet of Things (IoT) applications has contributed to the development of smart cities. A smart city utilizes IoT-enabled technologies, communications and applications to maximize operational efficiency and enhance both the service providers’ quality of services and people’s wellbeing and quality of life. With the growth of smart city networks, however, comes the increased risk of cybersecurity threats and attacks. IoT devices within a smart city network are connected to sensors linked to large cloud servers and are exposed to malicious attacks and threats. Thus, it is important to devise approaches to prevent such attacks and protect IoT devices from failure. In this paper, we explore an attack and anomaly detection technique based on machine learning algorithms (LR, SVM, DT, RF, ANN and KNN) to defend against and mitigate IoT cybersecurity threats in a smart city. Contrary to existing works that have focused on single classifiers, we also explore ensemble methods such as bagging, boosting and stacking to enhance the performance of the detection system. Additionally, we consider an integration of feature selection, cross-validation and multi-class classification for the discussed domain, which has not been well considered in the existing literature. Experimental results with the recent attack dataset demonstrate that the proposed technique can effectively identify cyberattacks and the stacking ensemble model outperforms comparable models in terms of accuracy, precision, recall and F1-Score, implying the promise of stacking in this domain. © 2020 by the authors. - Description: COVID-19 has spread all over the world, having an enormous effect on our daily life and work. In response to the epidemic, a lot of important decisions need to be taken to save communities and economies worldwide. Data clearly play a vital role in effective decision-making. Data-driven decision-making uses data-related evidence and insights to guide the decision-making process and verify the plan of action before it is committed. To better handle the epidemic, governments and policy-making institutes have investigated abundant data originating from COVID-19. These data include those related to medicine, knowledge, media, and so on. Based on these data, many prevention and control policies are made. In this survey article, we summarize the progress of data-driven decision-making in the response to COVID-19, including COVID-19 prevention and control, psychological counseling, financial aid, work resumption, and school reopening. We also propose some current challenges and open issues in data-driven decision-making, including data collection and quality, complex data analysis, and fairness in decision-making. This survey article sheds light on current policy-making driven by data, which also provides a feasible direction for further scientific research. © 2014 IEEE.
