- Description: The six cities air pollution is used to estimate and investigate the marginal curve of a function describing lung growth for set of children in a longitudinal study. This article proposes penalized regression spline technqiue based ona semiparametric mixed models (MM) framework for an additive model. This smoothing approach fits marginal models for longitudinal unbalanced measurements by using a Bayesian inference approach, implemented using a Markov chain Monte Carlo approach with the Gibbs sampler. The unbalanced case in which missing or different number of measurements for a set of subjects is more practical and common in real life studies. This methodology makes it possible to establish a straightforward approach to similar models using R programming, when it is not possible to do so using existing codes.

- Description: Most studies of rock slope failures on open–pit mines have considered either toppling or circular failure stability analysis. By comparison, complex circular–toppling failure has received much less attention in the published literature. This paper presents a study using a range of methods to investigate a failure that occurred in July 2008 in Handlebar Hill, an open–pit base metal mine, near Mt Isa, Australia. Circular failure is the typical slope failure mechanism in slopes with low–strength rocks, although direct/flexural toppling of jointed columns can also occur. The study reviews circular–toppling failure mechanisms in the context of the local geotechnical and geo–hydrological conditions, which include the interaction between fault contacts and the existing deformed rocks. General limit equilibrium methods are used to evaluate the sensitivity of slope models to rock strength parameters and the trigger mechanisms. Finite element methods are used to assess the failure mechanisms and slope displacement, and a kinematic approach is used to evaluate structurally controlled slope instability mechanisms. The results demonstrate that the most credible failure mechanism was shearing along a circular path through the upper weaker rocks (leached Magazine Shale) that in turn initiated secondary block toppling, and the progressive nature of the slope failure mechanism. The use of conventional and numerical techniques for back–analysis of the combined circular–toppling failure provided key insights into the failure mechanisms and factors controlling slope instability. © 2019, Springer Nature Switzerland AG.

- Description: Slopes in open pits exhibit fracturing around excavations, often initiated by extension strain which results from a combination of principal stresses adjacent to the slope boundaries. This extension strain is commonly described using minimum principal strain or minimum principal stress equations. These equations show that the extension strain can expand and fracturing occurs if the extension strain exceeds a critical value. Anisotropic rock masses with multiple and complex structures increase the potential of the development and coalesce of cracks with pre-existing discontinuities for further potential failure. This paper presents finite element analysis to model the extensions strain implementing the criterion of Stacey (1981). The distributions of extension strain are predicted around slope of Handlebar Hill open pit mine at Mt Isa, Queensland, Australia. Around the pit wall, fracturing near the excavation boundary is often the result of extension strain of the rock. Through the mining activities, fractures in the slope face can manifest into slabbing and spalling. Extension strain may develop circumferential fractures close to the slope surface, the closer to the excavation perimeter, the more open the cracks. The result of the extension strain distribution simulated in this paper is in accord with failure events observed on site. The numerical modelling and the discussion of this study focused on the prediction of potential fracturing zones within the critical values of the extension strain around the slope. © Springer Nature Switzerland AG 2020.

- Description: Rock characterisation is important to the feasibility of the Handlebar Hill open cut mine at Mt Isa, Queensland-Australia, because of the complex structural geology and the diversity of slope formations. The different rocks are affected by complete and moderate oxidisations coupled with mining works, giving rise to potential slope instability. Through the characterisation of these rocks, there is more confidence in the prediction of their behaviors in terms of failure mechanisms and slope stability. The objective of this research was to evaluate the properties of the pit rock masses. The geotechnical engineering practice approach was based on defining the parameters of the Hoek-Brown, Barton-Bandis and Mohr-Coulomb failure criteria. A program involving investigation that included field measurements, laboratory tests, hydrogeological settings, empirical indices and findings using the RocLab program was applied. The inputs help to analysis of pit slope stability and to understand the effects of different pit configurations on slope performance to allow safe and economic mining operations. © Springer Nature Switzerland AG 2020.

- Description: Rock Bridge fracturing and coalescence with pre-existing discontinuities in rock mass due to the initiation, propagation and interaction of these fractures refers to instability mode of step-path failure. Step-path failure is a typical type of instable mode of man-made and natural rock slopes. The continuum finite element method was applied to work on deeper insight into the propagation of tensile cracks which developing in the intact rock bridges that can finally coalesce to form step-path failure. In this paper, based on the intact rock fracturing hypothesis, two selected slope simulations from the Handlebar Hill open - pit mine near Mt. Isa in Queensland, Australia, modeled the process of fracturing and step-path failure through different pre-existing discontinuities. The empirical models of Bobet and Einstein (1998) and the progressively cracks development are observed within crack initiation, propagation and coalescence in the intact rock bridges. Proposed slope models of the mine included four joint-net distributions through the rock masses considering the geometry of structures (dip angles, spacing, lengths and orientation) illustrated the extension cracks from the flaw tips and propagated to the slope surface. Modes of intact rock bridges fracturing (shear, tensile and a combination of shear and tensile) have been observed. Tensile fracture is usually generated when the rock bridge angle is sub-vertical. Shear fracture can be initiated in less steep rock bridge angles. A combination of shear and tensile failure is normally generated in slopes with. Slope with explicit large-scale structures of steeper dip angles increased the yielding. Larger structures show much higher potential for yielding as the tensile stresses increasing. Major joint plane spacing resulted in less potential for relative deformations between neighboring structures and consequently reduced slope instability. The changes of length and spacing have more influence on slope stability than a change in the dip angle of the structures. © 2019, Springer Nature Switzerland AG.

- Description: Aims and objectives: To describe healthcare providers' attitudes to family involvement during routine care and family presence during resuscitation or other invasive procedures in adult intensive care units in Saudi Arabia. Background: Previous research has shown that healthcare professionals have revealed a diversity of opinions on family involvement during routine care and family presence during resuscitation or other invasive procedures. Attitude assessment can provide an indication of staff acceptance or rejection of the practice and also help identify key potential barriers that will need to be addressed. It has also been evident that participation in the care has potential benefits for patients and families as well as healthcare providers. Design: A quantitative descriptive design. Methods: A questionnaire was used with a convenience sample of 468 healthcare providers who were recruited from eight intensive care units. Results: The analysis found that healthcare providers had positive attitudes towards family involvement during routine care, but negative attitudes towards family presence during resuscitation or other invasive procedures. Physicians expressed more opposition to the practice than did nurses and respiratory therapists. Staff indicated a need to develop written guidelines and policies, as well as educational programmes, to address this sensitive issue in clinical practice. Conclusion: Family is an important resource in patient care in the context of the critical care environment. Clinical barriers including resources, hospital policies and guidelines, staff and public education should be taken into account to facilitate family integration to the care model. Relevance to clinical practice: The findings can help to develop policies and guidelines for safe implementation of the practice. They can also encourage those who design nursing and other medical curricula to place more emphasis on the role of the family especially in critical care settings. © 2013 John Wiley & Sons Ltd.

- Description: Aim: To identify the needs of families of adult intensive care unit (ICU) patients in Saudi Arabia as perceived by family members and health care providers. Background: Family members of critically ill patients are likely to have specific needs that should be addressed by the critical care team and which, if unmet, may produce stress for patients' families and health care providers. The literature has yet to identify the needs of Muslim families in relation to religious beliefs and cultural values in critical care settings in Saudi Arabia. Design: A cross-sectional survey design. Method: A total of 176 family members and 497 intensive health care providers were recruited from eight adult mixed medical-surgical ICUs between November 2011 and February 2012 utilizing a four-point Likert type scale self-administered questionnaire. Results: The findings revealed that family members and health care providers ranked assurance, information and cultural and spiritual needs as the most important, and support and proximity as least important. There were significant differences in the mean values found between family members and health care providers. A significant finding not identified in other studies was 'The need to have the health care providers handle the body of the dead Muslim with extreme caution and respect' which, under the dimension of cultural and spiritual needs, was perceived by family members to be the most important and by the health care providers as the fifth most important need. Conclusion: The recognition of family needs in the critical care unit informed the development of interventions to meet family needs and improve the care quality. © 2013 British Association of Critical Care Nurses.

- Description: Background: The presence of family in the patient care area during resuscitation events is a matter of current debate among health care professionals in many communities. Family presence is highly recommended by many health organizations worldwide for several reasons including patient and family rights. There are no policies or guidelines in Saudi Arabia to guide health professionals in their practice regarding the option of family being present during resuscitations. The purpose of this study was to identify the attitudes of nurses towards family presence during resuscitation in the Muslim community of Saudi Arabia. Design: This is a descriptive survey using data from a convenience sample of 132 nurses using a self-administered questionnaire. The study took place in two major trauma centres in the eastern region of Saudi Arabia. Results: The analysis of the data revealed that nurses (n = 132) had negative attitudes towards family presence during resuscitation. A high percentage (77·2%) agreed that witnessing resuscitation is a traumatic experience for the family members. Almost all participants (92·3%) disagreed with the statement that the practice of allowing family members to be present during the resuscitation of a loved one would benefit the patient and 78% disagreed with the statement that it would benefit families. The majority of the participants (65%) revealed that the presence of family would negatively affect the performance of the resuscitation team. However, almost half of the sample (43·8%) would prefer a written policy allowing the option of family presence during resuscitation in Saudi Arabia. Conclusion: The findings of the study strongly suggest the need for the development of written policies offering families the option to remain with patients during resuscitation in Saudi Arabia. The study further recommends the development of education programs for staff and public for the safe implementation of the practice. © 2012 The Authors. - Description: Aims and objectives: To understand the needs of critically ill patient families', seeking to meet those needs and explore the process and patterns of involving family members during routine care and resuscitation and other invasive procedures. Methods: A structured literature review using Cumulative Index to Nursing and Allied Health Literature, Pubmed, Proquest, Google scholar, Meditext database and a hand search of critical care journals via identified search terms for relevant articles published between 2000 and 2010. Results: Thirty studies were included in the review either undertaken in the Intensive Care Unit or conducted with critical care staff using different methods of inquiry. The studies were related to family needs; family involvement in routine care; and family involvement during resuscitation and other invasive procedures. The studies revealed that family members ranked both the need for assurance and the need for information as the most important. They also perceived their important needs as being unmet, and identified the nurses as the best staff to meet these needs, followed by the doctors. The studies demonstrate that both family members and healthcare providers have positive attitudes towards family involvement in routine care. However, family members and healthcare providers had significantly different views of family involvement during resuscitation and other invasive procedures. Conclusion: Meeting Intensive Care Unit family needs can be achieved by supporting and involving families in the care of the critically ill family member. More emphasis should be placed on identifying the family needs in relation to the influence of cultural values and religion held by the family members and the organisational climate and culture of the working area in the Intensive Care Unit. © 2013 Blackwell Publishing Ltd.
Hyperbolic smoothing in nonsmooth optimization and applications

- Description: Nonsmooth nonconvex optimization problems arise in many applications including economics, business and data mining. In these applications objective functions are not necessarily differentiable or convex. Many algorithms have been proposed over the past three decades to solve such problems. In spite of the significant growth in this field, the development of efficient algorithms for solving this kind of problem is still a challenging task. The subgradient method is one of the simplest methods developed for solving these problems. Its convergence was proved only for convex objective functions. This method does not involve any subproblems, neither for finding search directions nor for computation of step lengths, which are fixed ahead of time. Bundle methods and their various modifications are among the most efficient methods for solving nonsmooth optimization problems. These methods involve a quadratic programming subproblem to find search directions. The size of the subproblem may increase significantly with the number of variables, which makes the bundle-type methods unsuitable for large scale nonsmooth optimization problems. The implementation of bundle-type methods, which require the use of the quadratic programming solvers, is not as easy as the implementation of the subgradient methods. Therefore it is beneficial to develop algorithms for nonsmooth nonconvex optimization which are easy to implement and more efficient than the subgradient methods. In this thesis, we develop two new algorithms for solving nonsmooth nonconvex optimization problems based on the use of the hyperbolic smoothing technique and apply them to solve the pumping cost minimization problem in water distribution. Both algorithms use smoothing techniques. The first algorithm is designed for solving finite minimax problems. In order to apply the hyperbolic smoothing we reformulate the objective function in the minimax problem and study the relationship between the original minimax and reformulated problems. We also study the main properties of the hyperbolic smoothing function. Based on these results an algorithm for solving the finite minimax problem is proposed and this algorithm is implemented in GAMS. We present preliminary results of numerical experiments with well-known nonsmooth optimization test problems. We also compare the proposed algorithm with the algorithm that uses the exponential smoothing function as well as with the algorithm based on nonlinear programming reformulation of the finite minimax problem. The second nonsmooth optimization algorithm we developed was used to demonstrate how smooth optimization methods can be applied to solve general nonsmooth (nonconvex) optimization problems. In order to do so we compute subgradients from some neighborhood of the current point and define a system of linear inequalities using these subgradients. Search directions are computed by solving this system. This system is solved by reducing it to the minimization of the convex piecewise linear function over the unit ball. Then the hyperbolic smoothing function is applied to approximate this minimization problem by a sequence of smooth problems which are solved by smooth optimization methods. Such an approach allows one to apply powerful smooth optimization algorithms for solving nonsmooth optimization problems and extend smoothing techniques for solving general nonsmooth nonconvex optimization problems. The convergence of the algorithm based on this approach is studied. The proposed algorithm was implemented in Fortran 95. Preliminary results of numerical experiments are reported and the proposed algorithm is compared with an other five nonsmooth optimization algorithms. We also implement the algorithm in GAMS and compare it with GAMS solvers using results of numerical experiments.

E-cigarette use among male smokers in Al-Ahsa, Kingdom of Saudi Arabia : a cross-sectional study

- Description: E-cigarette use is increasing globally. Recent evidence suggests that e-cigarettes contain harmful substances that could cause adverse health outcomes. This study investigated the prevalence and associated factors of e-cigarette use among male current smokers in Saudi Arabia. We conducted a cross-sectional survey of adult male current smokers in the Al-Ahsa province of Saudi Arabia. Data were collected using a structured questionnaire. We performed logistic regression analyses to investigate the factors associated with e-cigarette use among adult male current smokers. 325 current smokers participated in the study. A third of them (33.5%) were e-cigarette users. Almost all the study participants (97.0%) had heard about e-cigarettes. Participants who were occasional smokers (Odds Ratio (OR): 2.28; 95% Confidence Interval (CI): 1.17–4.41) and had good knowledge perception of e-cigarettes (OR 3.49; 95% CI: 2.07–5.90) had higher odds of using e-cigarettes when compared to regular smokers of conventional cigarettes and current smokers with poor knowledge perception of e-cigarettes, respectively. In contrast, private employees (OR: 0.25, 95% CI: 0.07–0.85), and business owners (OR: 0.09, 95% CI: 0.01–0.63) had lower odds of using e-cigarettes compared to unemployed individuals. Compared with non-e-cigarette users, the rate of conventional cigarette smoking per day was significantly lower among e-cigarette users. Use of e-cigarette (OR: 3.57, 95% CI: 2.14–5.98), believing that e-cigarette quitting is hard (OR: 2.02, 95% CI: 1.17–3.49) and trying to quit e-cigarettes (OR: 2.17, 95% CI: 1.1–4.25) were found to be significant predictors of good knowledge perception of e-cigarettes among the current smokers. The use and knowledge perception of e-cigarettes were higher among occasional conventional male cigarette smokers than regular male smokers in Al-Ahsa province. The use of e-cigarettes as smoking cessation aids should be examined further in the Saudi Arabian setting. © 2022 by the authors.

- Description: E-cigarette use is increasing globally. Recent evidence suggests that e-cigarettes contain harmful substances that could cause adverse health outcomes. This study investigated the prevalence and associated factors of e-cigarette use among male current smokers in Saudi Arabia. We conducted a cross-sectional survey of adult male current smokers in the Al-Ahsa province of Saudi Arabia. Data were collected using a structured questionnaire. We performed logistic regression analyses to investigate the factors associated with e-cigarette use among adult male current smokers. 325 current smokers participated in the study. A third of them (33.5%) were e-cigarette users. Almost all the study participants (97.0%) had heard about e-cigarettes. Participants who were occasional smokers (Odds Ratio (OR): 2.28; 95% Confidence Interval (CI): 1.17–4.41) and had good knowledge perception of e-cigarettes (OR 3.49; 95% CI: 2.07–5.90) had higher odds of using e-cigarettes when compared to regular smokers of conventional cigarettes and current smokers with poor knowledge perception of e-cigarettes, respectively. In contrast, private employees (OR: 0.25, 95% CI: 0.07–0.85), and business owners (OR: 0.09, 95% CI: 0.01–0.63) had lower odds of using e-cigarettes compared to unemployed individuals. Compared with non-e-cigarette users, the rate of conventional cigarette smoking per day was significantly lower among e-cigarette users. Use of e-cigarette (OR: 3.57, 95% CI: 2.14–5.98), believing that e-cigarette quitting is hard (OR: 2.02, 95% CI: 1.17–3.49) and trying to quit e-cigarettes (OR: 2.17, 95% CI: 1.1–4.25) were found to be significant predictors of good knowledge perception of e-cigarettes among the current smokers. The use and knowledge perception of e-cigarettes were higher among occasional conventional male cigarette smokers than regular male smokers in Al-Ahsa province. The use of e-cigarettes as smoking cessation aids should be examined further in the Saudi Arabian setting. © 2022 by the authors.

- Description: Background: All nurses must have core competencies in preparing for, responding to and recovering from a disaster. In the Kingdom of Saudi Arabia (KSA), as in many other countries, disaster nursing core competencies are not fully understood and lack reliable, validated tools. Thus, it is imperative to develop a scale for exploring disaster nursing core competencies, roles and barriers in the KSA. Objectives: This study's objective is to develop a valid, reliable scale that identifies and explores core competencies of disaster nursing, nurses' roles in disaster management and barriers to developing disaster nursing in the KSA. Methods: This study developed a new scale testing its validity and reliability. A principal component analysis (PCA) was used to develop and test psychometric properties of the new scale. The PCA used a purposive sample of nurses from emergency departments in two hospitals in the KSA. Participants rated 93 paper-based, self-report questionnaire items from 1 to 10 on a Likert scale. PCA using Varimax rotation was conducted to explore factors emerging from responses. Findings: The study's participants were 132 nurses (66% response rate). PCA of the 93 questionnaire items revealed 49 redundant items (which were deleted) and 3 factors with eigenvalues of >1. The remaining 44 items accounted for 77.3% of the total variance. The overall Cronbach's alpha was 0.96 for all factors: 0.98 for Factor 1, 0.92 for Factor 2 and 0.86 for Factor 3. Conclusions: This study provided a validated, reliable scale for exploring nurses' core competencies, nurses' roles and barriers to developing disaster nursing in the KSA. The new scale has many implications, such as for improving education, planning and curricula. © 2015.

- Description: Summary Background It is generally accepted that nurses have insufficient knowledge about disaster preparedness due to a lack of acceptance of core competencies and the absence of disaster preparedness in nursing curricula.1 This study explored nurses’ knowledge and sources of knowledge, and skills as they relate to disaster management in Saudi Arabia, where more than 4660 people have died, 32,000 people have been affected, and US$4.65 billion in damage has been caused by disaster since 1980.2 Methods A quantitative, non-experimental, descriptive research design. Results Nurses in Saudi Arabia have moderate knowledge concerning disaster preparedness. However, nurses in military hospitals possess more knowledge than those who work in government hospitals. The majority of nurses gained their knowledge and skills from disaster drills. Conclusions Nurses need more education in all areas of disaster management, most importantly in their roles during response to disasters. Nurses perceive themselves as not well-prepared but they are willing to improve their skills in disaster preparedness if educational opportunities are provided.

- Description: Obstacles present in the line of sight transmission path of a wireless signal severely attenuates the received signal power. Extreme fluctuations of the received signal power caused by shadowing can create "blind spots". Blind spots are areas within the cellular coverage area from where no communication is possible to the base station (BS). For single path packet based services link failures results in total loss of communication. The problem of link failure and blind spots can almost be eliminated in the recently proposed multipath cellular architecture (MCA) which provisions up to three different communication links from the mobile node (MN) to three adjacent BSs through overlapped coverage. Voice and/or multimedia packet delivery can thus benefit by establishing communication from the best interface in terms of received signal power. In this paper, we evaluate the performance of the multi-path MCA model in reducing link failure and blind spot communication problems in presence of multiple obstacles and shadow fading. Obstacle mobility model is utilized as the preferred user mobility model. Video transmission performance improves significantly in terms of reduced packet loss and improved reproduced signal quality at the receiver for the multipath MCA model compared to the existing single path cellular network architecture

Based on diagnoses made by an automated online assessment and diagnostic system administered to a large group of online participants, multiple comorbidities (co-occurrences) of 21 psychological disorders for males and females were identified. We examined the relationships between dyadic comorbidities of anxiety and depressive disorders and the psychosocial variables sex, age, suicidal ideation, social support, and quality of life. Methods: An online complex algorithm based on the criteria of the Diagnostic and Statistical Manual of Mental Disorders, 4th edition, Text Revision, was used to assign primary and secondary diagnoses of 21 psychological disorders to 12,665 online participants. The frequency of co-occurrences of psychological disorders for males and females were calculated for all disorders. A series of hierarchical loglinear analyses were performed to examine the relationships between the dyadic comorbidities of depression and various anxiety disorders and the variables suicidal ideation, social support, quality of life, sex, and age. Results: A 21-by-21 frequency of co-occurrences of psychological disorders matrix revealed the presence of multiple significant dyadic comorbidities for males and females. Also, for those with some of the dyadic depression and the anxiety disorders, the odds for having suicidal ideation, reporting inadequate social support, and poorer quality of life increased for those with two-disorder comorbidity than for those with only one of the same two disorders. Conclusions: Comorbidities of several psychological disorders using an online assessment tool within a Web-based population were similar to those found in face-to-face clinics using traditional assessment tools. Results provided support for the transdiagnostic approaches and confirmed the positive relationship between comorbidity and suicidal ideation, the negative relationship between comorbidity and social support, and the negative relationship comorbidity and quality of life. © 2015, Journal of Medical Internet Research. Posttreatment attrition and its predictors, attrition bias, and treatment efficacy of the anxiety online programs

- Description: Background: Although relatively new, the field of e-mental health is becoming more popular with more attention given to researching its various aspects. However, there are many areas that still need further research, especially identifying attrition predictors at various phases of assessment and treatment delivery. Objective: The present study identified the predictors of posttreatment assessment completers based on 24 pre- and posttreatment demographic and personal variables and 1 treatment variable, their impact on attrition bias, and the efficacy of the 5 fully automated self-help anxiety treatment programs for generalized anxiety disorder (GAD), social anxiety disorder (SAD), panic disorder with or without agoraphobia (PD/A), obsessive-compulsive disorder (OCD), and posttraumatic stress disorder (PTSD). Methods: A complex algorithm was used to diagnose participants' mental disorders based on the criteria of the Diagnostic and Statistical Manual of Mental Disorders (Fourth Edition, Text Revision; DSM-IV-TR). Those who received a primary or secondary diagnosis of 1 of 5 anxiety disorders were offered an online 12-week disorder-specific treatment program. A total of 3199 individuals did not formally drop out of the 12-week treatment cycle, whereas 142 individuals formally dropped out. However, only 347 participants who completed their treatment cycle also completed the posttreatment assessment measures. Based on these measures, predictors of attrition were identified and attrition bias was examined. The efficacy of the 5 treatment programs was assessed based on anxiety-specific severity scores and 5 additional treatment outcome measures. Results: On average, completers of posttreatment assessment measures were more likely to be seeking self-help online programs; have heard about the program from traditional media or from family and friends; were receiving mental health assistance; were more likely to learn best by reading, hearing and doing; had a lower pretreatment Kessler-6 total score; and were older in age. Predicted probabilities resulting from these attrition variables displayed no significant attrition bias using Heckman's method and thus allowing for the use of completer analysis. Six treatment outcome measures (Kessler-6 total score, number of diagnosed disorders, self-confidence in managing mental health issues, quality of life, and the corresponding pre- and posttreatment severity for each program-specific anxiety disorder and for major depressive episode) were used to assess the efficacy of the 5 anxiety treatment programs. Repeated measures MANOVA revealed a significant multivariate time effect for all treatment outcome measures for each treatment program. Follow-up repeated measures ANOVAs revealed significant improvements on all 6 treatment outcome measures for GAD and PTSD, 5 treatment outcome measures were significant for SAD and PD/A, and 4 treatment outcome measures were significant for OCD. Conclusions: Results identified predictors of posttreatment assessment completers and provided further support for the efficacy of self-help online treatment programs for the 5 anxiety disorders

However, there are many gaps in the research. One such gap is in the area of attrition predictors at various stages of assessment and treatment delivery. Objective: This exploratory study applied univariate and multivariate analysis to a large dataset provided by the Anxiety Online (now called Mental Health Online) system to identify predictors of attrition in treatment commencers and in those who formally withdrew during treatment based on 24 pretreatment demographic and personal variables and one clinical measure. Methods: Participants were assessed using a complex online algorithm that resulted in primary and secondary diagnoses in accordance with the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR). Those who received a primary or secondary diagnosis of 1 of 5 anxiety disorders (generalized anxiety disorder, social anxiety disorder, obsessive-compulsive disorder, posttraumatic stress disorder, and panic disorder) were offered an online 12-week disorder-specific treatment program. Results: Of 9394 potential participants, a total of 3880 clients enrolled and 5514 did not enroll in one of the treatment programs following the completion of pretreatment assessment measures (pretreatment attrition rate: 58.70%). A total of 3199 individuals did not formally withdraw from the 12-week treatment cycle, whereas 142 individuals formally dropped out (formal withdrawal during treatment dropout rate of 4.25%). The treatment commencers differed significantly (P<.001-.03) from the noncommencers on several variables (reason for registering, mental health concerns, postsecondary education, where first heard about Anxiety Online, Kessler-6 score, stage of change, quality of life, relationship status, preferred method of learning, and smoking status). Those who formally withdrew during treatment differed significantly (P=.002-.03) from those who did not formally withdraw in that they were less likely to express concerns about anxiety, stress, and depression; to rate their quality of life as very poor, poor, or good; to report adequate level of social support; and to report readiness to make or were in the process of making changes. Conclusions: This exploratory study identified predictors of pretreatment attrition and formal withdrawal during treatment dropouts for the Anxiety Online program.

Those who formally withdrew during treatment differed significantly (P=.002-.03) from those who did not formally withdraw in that they were less likely to express concerns about anxiety, stress, and depression; to rate their quality of life as very poor, poor, or good; to report adequate level of social support; and to report readiness to make or were in the process of making changes. Conclusions: This exploratory study identified predictors of pretreatment attrition and formal withdrawal during treatment dropouts for the Anxiety Online program
Comorbidity structure of psychological disorders in the online e-PASS data as predictors of psychosocial adjustment measures: psychological distress, adequate social support, self-confidence, quality of life, and suicidal ideation

- Description: BACKGROUND: A relative newcomer to the field of psychology, e-mental health has been gaining momentum and has been given considerable research attention. Although several aspects of e-mental health have been studied, 1 aspect has yet to receive attention: the structure of comorbidity of psychological disorders and their relationships with measures of psychosocial adjustment including suicidal ideation in online samples. OBJECTIVE: This exploratory study attempted to identify the structure of comorbidity of 21 psychological disorders assessed by an automated online electronic psychological assessment screening system (e-PASS). The resulting comorbidity factor scores were then used to assess the association between comorbidity factor scores and measures of psychosocial adjustments (ie, psychological distress, suicidal ideation, adequate social support, self-confidence in dealing with mental health issues, and quality of life). METHODS: A total of 13,414 participants were assessed using a complex online algorithm that resulted in primary and secondary Diagnostic and Statistical Manual of Mental Disorders (Fourth Edition, Text Revision) diagnoses for 21 psychological disorders on dimensional severity scales. The scores on these severity scales were used in a principal component analysis (PCA) and the resulting comorbidity factor scores were related to 4 measures of psychosocial adjustments. RESULTS: A PCA based on 17 of the 21 psychological disorders resulted in a 4-factor model of comorbidity: anxiety-depression consisting of all anxiety disorders, major depressive episode (MDE), and insomnia; substance abuse consisting of alcohol and drug abuse and dependency; body image-eating consisting of eating disorders, body dysmorphic disorder, and obsessive-compulsive disorders; depression-sleep problems consisting of MDE, insomnia, and hypersomnia. All comorbidity factor scores were significantly associated with psychosocial measures of adjustment (P<.001). They were positively related to psychological distress and suicidal ideation, but negatively related to adequate social support, self-confidence, and quality of life. CONCLUSIONS: This exploratory study identified 4 comorbidity factors in the e-PASS data and these factor scores significantly predicted 5 psychosocial adjustment measures. Addition of activated carbon into a cattle diet to mitigate GHG emissions and improve production

- Description: Globally, the most problematic greenhouse gas (GHG) emissions of ruminant livestock is methane (CH4), with a global warming potential 25 times that of carbon dioxide. This work considers the emissions and production effects of powdered activated carbon (PAC) at 0.5% by dry matter (DM) on methanogenic rumen flora as the major source of dairy cattle enteric methane emissions. In total, 180 dairy cattle located in Brymaroo, Queensland (QLD), Australia, were studied in a three-cycle repeated measures ANOVA format with a 4 week primary interval. Emissions eructated during milking and in faecal deposits were measured, and in addition, 16S rRNA gene sequencing was performed to determine the collective populations of prokaryotic bacteria and archaea as well methanogenic communities for each treatment. Moreover, 0.5% PAC addition reduced CH4 emissions by 30-40% and CO2 emissions by 10%, while improving daily milk production by 3.43%, milk protein by 2.63% and milk fat by 6.32%, on average for the herd (p < 0.001 in all cases). rRNA gene sequencing showed populations of methanogenic flora decreased by 30% on average with a corresponding increase in the nonmethanogenic species. We strongly advocate further on-farm trials with the dietary addition of PAC in ruminant diets to mitigate emissions while maintaining or improving productivity. © 2021 by the authors.
